Description
Statement of Work for W912J7-13-T-0005 Medical Surveillance Examinations Synopsis: This is a combined synopsis/solicitation prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and written solicitation will not be issued. This solicitation is issued under Request for Quotation (RFQ) number W912J7-12-T-0015. This solicitation incorporates provisions and clauses for commercial items in effect through Federal Acquisition Circular FAC 2005-60 dated 27 August 2012. The associated North American Industry Classification system code (NAICS) for this procurement is 622110 with a small business size standard of $34.5 million. This requirement is unrestricted and only qualified offerors may submit bids. The Idaho Army National Guard will be awarding a Firm Fixed Price for the following requirement. The Idaho Army National Guard (IDARNG) Occupational Health Services has a requirement for Medical Surveillance Examinations for its full time civilian federal technician workforce in selected maintenance areas. No examinations will be provided to State of Idaho employees, civilian contractors, and IDARNG Active Guard/Reserve (AGR) or part time soldiers. The contractor shall provide all personnel, supervision, and any items and services necessary to perform the services. The Government will evaluate the quality of professional and administrative services provided, but retain no control over the medical professional aspects of services rendered such as testing procedures, etc. Government evaluation criteria are identified in Technical Exhibit 1, Performance Requirements Summary. Contractors will provide all of the examinations as defined in the Performance Work Statement attachment of this combined synopsis/solicitation. All examinations will occur within Boise Idaho City limits, no other examination location will be acceptable. An award will be made based on LPTA (lowest price technically acceptable) in accordance with FAR Subpart 15.101-2. The following must be submitted in order to determine technical acceptability of a contractor; 1. A statement certifying that the offeror can perform all of the aforementioned services and equipment in accordance with the Performance Work Statement for services to the Idaho Army National Guard. 2. A listing of all personnel that will perform services as outlined in section 4.0 Specific Tasks of the Performance Work Statement that require certification. The listing must include the type of certification and certification number of the personnel that will conduct the services. Certifications requiring documentation are; Occupational Health Board Certified physician (PWS section 4.2), American Society of Clinical Pathology (PWS section 4.3), Licensed physician, Certified Pulmonary Function Technologist, or Registered Respiratory Therapist (PWS section 4.4), Certified X-Ray technician (PWS section 4.7), B-reader, board eligible, or certified radiologist (PWS section 4.7). 3. Technical acceptability will be determined as a Go or No Go. Past performance will be evaluated based on the correlation of past performance to the requirement as outlined in section 4.0 Specific Tasks of the Performance Work Statement. The following questionnaire must be submitted in order to determine past performance acceptability of a contractor; 1. What are the names, locations, and contact information for the last two Government entities/activities that similar services were contracted to? Include date ranges, percent completed, and total contracted dollar amount for each contracting action identified. 2. If any of the contracting actions referenced above utilized subcontracting, to what extent did your firm attain applicable goals for small business participation under contracts that required subcontracting plans? 3. What types of services where rendered to the above identified Government entities/activities? 4. How does your organizations past performance relate to your ability to satisfy the Specific Tasks section 4.0 of the Performance Work Statement? All prospective contractors interested in submitting a quote on this combined synopsis/solicitation must be registered on System for Award Management (SAM) database. Effective 21 August 2012 SAM replaced Central Contractor Registration (CCR) database. If the contractor was previously registered in CCR then the registration should have automatically migrated to SAM, however it would be prudent to ensure the contractors data successfully migrated to SAM as a lack of registration in SAM will make a contractor ineligible for award. The website for SAM registration verification is https://www.sam.gov/portal/public/SAM. The contractor must also be registered on Wide Area Work Flow (WAWF), the Army's electronic invoicing system, prior to invoice submission. The website to register in WAWF is: www.wawf.eb.mil. This combined synopsis/solicitation will close 25 September 2013 at 11:59pm EST.
