CURRENT DOL WAGE DETERMINATION 05-2271 Rev 13 MANDATORY TO COMPLETE AND RETURN WITH QUOTE - Price Sheet MANDATORY TO COMPLETE AND RETURN WITH QUOTE - AGAR and Reps and Certs 52.232-99 Clause Spec 82313 - Specifications (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) The solicitation number is AG-569R-S-13-0053. This solicitation is issued as a request for quotations (RFQ) for a Firm Fixed Price contract. Simplified procurement procedures will be used for this acquisition per Federal Acquisition Regulation (FAR) Part 13. The Government intends to make one contract award. (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-69. Provisions and clause incorporated by reference have the same force and effect as if they were given in full text. The associated NAICS code is 561720 and the small business size standard is $16.5M. (v) Provide janitorial services for the USDA Huron-Manistee National Forest, Huron Shores Ranger Station office building and fire annex located in Oscoda, Michigan. (vi) Description of requirements for the items to be acquired: Firm-fixed price, non-personal services to provide all personnel, equipment, tools, supervision and other items necessary to perform janitorial services for the USDA Huron-Manistee National Forest, Huron Shores Ranger Station office building and fire annex located in Oscoda, Michigan. SEE SPECS 82313 ATTACHED (vii) The start date will be on November 1, 2013. Period of performance is for one base year of November 1, 2013 - October 31, 2014. There are four (4) option years that may extend the contract through yearly intervals until October 31, 2018. Place of performance is located Huron Shores Ranger Station of the Huron-Manistee National Forest. The district is located in the Township of Oscoda, Iosco County, Michigan. (viii) Provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. (ix) Evaluation of quotes will be based on the Best Value to the Government as determined by qualifications and experience, past performance, and price. Evaluation Factors below shall be evaluated as all being of equal importance to the Government. (1) Qualifications and experience of the crew(s) and any proposed subcontractors. (2) Past performance on three (3) contracts with Government agencies and private clients in terms of cost control, quality of work and delivery schedules (3) Price Evaluation/Fair and reasonable Contractors shall submit statements of qualifications and experience for personnel that will be providing the services and detailed price quote for each year and option year. Please provide a breakdown of how the monthly price was calculated. Contractors that fail to respond accordingly may be considered non-responsive and may not be considered for award. (x) Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items (attachment available in uploaded document package), with its quote. An offeror shall complete only paragraphs (b) of this provision if the offeror has completed the annual representations and certificates electronically via the System for Award Management (SAM) online at www.sam.gov. If an offeror has NOT completed the annual representations and certifications electronically at the online, the offeror shall complete only paragraphs (c) through (o) of this provision.
