- Updated Section D, Eval Factors for Award Attachment 3, PPQ Attachment 2, Offeror Experience Project Data Sheet Attachment 1, CLINS Schedule and Exhibits A-E SOW REV-2 13 Sep 2013 AMENDMENT 1 A) The purpose of this amendment is to incorporate the following changes: a. The proposal due date has been extended from 17 September 2013 to 19 September 2013 (Thursday) and must be received by this office no later than 2:00 PM GUAM Standard Time. b. Statement of Work (SOW) is replaced in its entirety with an updated SOW dated 12 Sep 13 included as attachment in this amendment. c. Attachment 1 (Exhibits A thru E) is replaced in its entirety with an updated Attachment 1 (Schedule and Exhibits A thru E) included as an attachment in this amendment. d. Section D, Evaluation Factors for Award, updated Section D is included as an attachment in this amendment. B) The following have been modified: INSTRUCTIONS TO OFFERORS 1. All responsible contractors are encouraged to submit an offer in response to this solicitation. Proposals must be received by this office no later than 2:00 p.m. (Guam Time) on Thursday, 19 September 2013. Proposals may be submitted electronically to NAVFAC Marianas FEAD Supply and Non-FSC Services, Attn: Jeno Aguon, Contract Specialist via email at jeno.aguon@fe.navy.mil or Dante P. Serneo, Contracting Officer at dante.serneo@fe.navy.mil. Note: Large file attachments may delay receipt of electronic mail. Offerors are responsible to ensure that proposals are entirely received by the due date and time required. Written offers (oral offers will not be accepted) may also be submitted to the following address: For Hand Delivery: NAVAL FACILITIES ENGINEERING COMMAND MARIANAS, BLDG 103 1ST Floor, MARINE DRIVE, NAVAL STATION, GUAM. You will need to call a Government POC upon arrival to the building to gain access. Submission Requirements: Offerors must submit, as part of their proposal, the following: (1) Complted Price Proposal: Attachment 1 (Schedule and Exhibits A thru E). Proposal must include pricing for CLINs 0001 thru 0005, to include pricing for the respective Exhibits A thru E. (2) Offeror Experience: Attachment 2, Completed Offeror Experience Project Data Sheet (no more than 5 projects). (3) Past Performance: Attachment 3, Completed Past Performance Questionnaire (PPQ) or CPAR report (no more than 5 projects). (4) Safety Factor: 1) Experience Modification Rate (EMR): For the three previous complete calendar years, 2010, 2011 and 2012; 2) OSHA Days Away from Work, Restricted Duty, or Job Transfer (DART) Rate: For the three previous complete calendar years 2010, 2011 and 2012 and; 3) Safety plan (Narrative shall be limited to two pages). (5) Completed copies of FAR 52.212-3, as well as DFARS 252.212-7000 or Online Representations and Certifications Application (ORCA) record. ORCA is located at https://orca.bpn.gov. The provisions and clauses can be found at FAR website: http://farsite.hill.af.mil and DFARS website: If you are submitting a project that your firm performed as a subcontractor, the prime contractor may be cited as the client POC. Additionally, the Government may review any other sources of information for evaluating past performance. Other sources may include, but are not limited to, past performance information retrieved through the Past Performance Information Retrieval System (PPIRS) using all CAGE/DUNS numbers of team members (partnership, joint venture, teaming arrangement, or parent company/subsidiary/affiliate) identified in the offeror's proposal, inquiries of owner representative(s), and any other known sources not provided by the offeror. Offerors who submit past performance and experience information of a parent, sister, predecessor or other affiliated company must describe what involvement such company will have on the instant acquisition. The proposal should clearly indicate how the resources of the parent, sister, predecessor or other affiliated company-its workforce, management, facilities, or other resources will be provided or relied upon for contract performance. To be eligiblle to receive an award from this solicitation. Offerors are required to provide their Contractors' DUNS Number with their proposal. Offeror must (No Exceptions) be registered in the System for Award Management (SAM) Registration Data Base. This acquisition is unrated in accordance with the Defense Priorities and Allocation System.
