Description

This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in Subpart 12.6, and is being conducted under FAR Part 13; simplified acquisition procedures apply. VA will conduct the procurement according to a combination of FAR Par 12, Acquisition of Commercial Items and Part 13, Simplified Acquisition Procedures as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a written solicitation will not be issued.. (ii) The solicitation number is VA256-13-Q-2139 and is issued as a Request For Quotation (RFQ) (iii) The provisions and clauses incorporated into this solicitation document are those in effect through Federal Acquisition Circular 2005-69, August 1, 2013. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The full text of the Federal Acquisition Regulations (FAR) and Veterans Affairs Acquisition Regulations supplement (VAAR) can be accessed on the Internet at http://www.arnet.gov/far (FAR) and http://vaww.appc1.va.gov/oamm/vaar (VAAR) (iv) This solicitation is issued as full and open competition. The North American Industry Classification System (NAICS) code is 334510, and the size standard is 500 employees. (v) This requirement consists of two (2) line items: CLIN (1) Acculis MTA System (or equal); QTY 1ea CLIN (2) Operators Manual; QTY 1ea (vi) Contractor to provide a Microwave Tissue Ablation System brand name or equal to an Acculis MTA System. The product offered must meet or exceed the following minimal salient characteristics: Applicator available in 14cm, 19cm and 29 cm shaft lengths; generator powers from 60w to 140w in 20w intervals, 2.45GHz operating frequency, touch screen user interface; cart with articulating arm. (vii) Delivery FOB Destination to Central Arkansas Healthcare System, 4300 West 7th Street, Little Rock, AR 72205 (viii) 52.212-1 Instructions to Offerors - commercial Items, apply to this solicitation with the following addenda, FAR 52.209-5DEV; 52.225-25; VAAR 852-215-70 (ix) 52.212-2, Evaluation - Commercial Items: FAR provision 52.212-2 does not apply to this solicitation. Pursuant to FAR subpart 13.106-2, the Government will award a firm fixed price contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Offerors shall submit with their quotation technical documentation that substantiates the offered product's ability to meet or exceed the minimum specifications of items identified within the schedule (paragraph (v)). (x) 52.212-3 Offeror Representations and Certifications - Commercial Items- An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically via https://www.acquisition.gov.
