- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and proposals are being requested. A separate solicitation will not be issued. The solicitation number is 260-13-Q-1230 and is issued as a Request for Proposals (RFP), unless otherwise indicated herein. The associated North American Industrial Classification System (NAICS) code for this procurement is 624221 with a small business size standard of $10M. This solicitation starts 09/18/2013 and will end 09/25/2013 at 12:00 PM PST. The Department of Veterans Affairs Roseburg VA Medical Center, hereafter referred to as RVAMC, has a need for four (4) reserved beds located within a homeless shelter in Eugene, OR. This is a Fixed Price, Base Year with four (4) one year options requirement. SCOPE OF WORK: This contract is for a Health Care for Homeless Veterans (HCHV) Interim Housing. The contractor shall provide transitional housing and supportive services for Veterans/Beneficiaries of RVMC. The contract facility must have a current occupancy permit by the authority having jurisdiction. The contractor shall furnish each Veteran authorized care under this contract with the following basic services: a.Residential Room and Board to include bed with linens and bathing facilities; b.Provide food for two balanced meals daily and snacks daily to each veteran resident. In order to support recovery plans, Veterans may be allowed to prepare their own meals with on-site staff supervision; c.Contractor to provide personal hygiene items, first aid supplies, and limited storage as needed; d.Laundry facilities and laundry supplies (i.e. detergent) for residents to do their own laundry; e.Supportive Services determined to be needed by the individual resident in a plan developed by the contractor with consultation by the veteran and the VA case manager and/or other appropriate VA staff. Services which the contractor must be able to furnish include; f.Structured group activities as appropriate - examples include, social skills training, Alcoholics Anonymous, Narcotics Anonymous, harm reduction and physical activities as appropriate; g.Communication and collaboration with the VA program staff, which will provide supportive psychosocial services; h.Life skills training including counseling on self-care skills, adaptive coping skills and, as appropriate, collaboration with VA program and community resources; i.Assistance to develop responsible living patterns and to achieve a more adaptive level of psychosocial functioning, upgraded social skills, and improved personal relationships; support for an alcohol/drug abuse-free lifestyle; j.Assistance to gain and to apply knowledge of the illness/recovery process. The selected Offeror shall comply with the following commercial item terms and conditions, which are incorporated herein by reference: FAR 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition; FAR 52.212-3, Offeror Representations and Certifications - Commercial Items - the selected offeror shall submit a completed copy of the listed representations and certifications; FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items.
