- The purpose of this amendment is to edit the Performance Work Statement (PWS). 1) (JCAHO) has changed to (JC) in sections 3, 7.3.5, 7.3.6, 7.4, and 8.4.3. 2) (HIPPA) has been corrected to (HIPAA) in section 7.4. 3) Health Insurance Portability and Accountability Act of 1996 (HIPAA) Requirement has been added in section 7.4.1. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Quotes are being requested a written solicitation will not be issued. Quotes shall reference solicitation number W911RZ-13-T-0121. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-69, dated 3 September 2013. Fort Carson intends to award a firm-fixed-price contract for Catholic Chaplain Services. The services shall be performed in accordance with the Performance Work Statement (PWS). A priest wishing to serve as a contract chaplain with the Archdiocese Military Service (AMS), must apply for and receive from the Military Ordinary the appropriate endorsement before AMS faculties may be granted and provide written proof to the Contracting Officer of permission from his diocesan bishop or religious superior to enter into these services as well as provide current Ecclesiastical Endorsement from the Archdiocese for Military Services (AMS), USA http://www.milarch.org prior to the performance start date. The period of performance (POP) is 01 October 2013 through 30 September 2014. The North American Industrial Classification System (NAICS) code for this procurement is 813110. This requirement is Unrestricted.
