MEADE, MARYLAND C1 GENERAL: C.1 The Fort George G. Meade Depart of Public Works (DPW) and the designated Contracting Officer's Representative (COR) will work with the contractor to manage the results achieved by this contract. The COR will be designated by the Contracting Officer (KO) in writing. C1.1 Scope of Work: The Contractor shall provide all labor, supervision, tools, equipment, supplies and materials necessary to perform schedule preventative maintenance, inspection, service calls (routine and emergency) and incidental repairs as specified for elevators and dumbwaiters at various locations on Fort Meade Maryland under the provisions of this contract. C.1.2. Liability of the Contractor: The contractor shall be responsible for full elevator maintenance service to comply with national Elevator Industry Inc. (NEII) part 7 Maintenance Guidelines, ASME A17.1 maintenance requirements, ASME A18.1 code inspections and tests, elevator performance measures, repairs and call backs for all elevators and dumbwaiters. The intent of this contract is to have all the responsibility for operation, management and supervision, and whenever possible, the majority of repairs and preventive maintenance accomplished by prime Contractor personnel. However, if the Contractor identifies a repair or service outside the skill sets of in house personnel, the Contractor shall provide written justification in advance, to the Contracting Officer (KO), Contracting Officer's Representative (COR), or designated government representative, for approval of the need to use a subcontractor. The use of subcontractors must adhere to the security clearance requirement of this solicitation set forth in this contract. The Contractor shall not use subcontractors to perform repairs unnecessarily or with the intent of driving up the repair cost. C.1.3. Working Hours: The contractor shall perform all scheduled maintenance, inspection and repair service during normal duties hours of 0730 and 1200 (7:30 AM through 12:00 AM) and between the hours of 1300 and1700 (1:00 PM through 5:00 PM) Monday through Friday. Emergency repair services shall occur outside of these hours and on Saturdays, Sundays and all Federal holidays. The Contractor shall contact the Directorate of Public Works (DPW) Work Control Center, the COR or their representative when first arriving at the facility. At the completion of work the Contractor employee shall submit a copy of the work order, or repair order to the COR. C.1.4 Site Inspections: Prospective bidders are encouraged to make a site inspection to determine the location of the elevators and dumbwaiters in order to satisfy themselves that they are aware of the exact nature of the work to be performed. C.1.5 Contractor Employee Competence and Demeanor: The Contractor shall ensure that all employees are knowledgeable in the maintenance and repair of the different manufacturers of Elevator, Dumbwaiter and Handicap Lift equipment necessary in order to completely perform specified tasks. The Contractor's employees shall be trained in order to competently perform the work. 1. Contractor personnel who enter the installation for routine or emergency work shall obey Government rules, regulations, directives, and requirements that are in effect or issued during the contract term governing law and order, administration, and security on the installation. The Contractor expressly agrees to remove from the site any individual whose continued employment is deemed by the Contracting Officer and Base Security to be contrary to the public interest or inconsistent with the best interest of national security. 2. The Contractor shall not allow any employee who is under the influence of alcohol, drugs, or any other incapacitating agents to perform work under this contract. Contract personnel shall not use Government equipment or facilities and shall not loiter on the grounds of Fort Meade. C.1.6. Contractor Quality Control and Government Quality Assurance: The Contractor shall implement plans for meeting the requirements of this contract. All plans shall include procedures and controls by which the Contractor shall accomplish the required tasks and evaluate the performance of his workforce. The plans shall be kept current as the Contractor's procedures are modified. The Contractor shall fully implement plans and procedures and subsequent modifications as approved by the Contracting Officer. Plans for work management, reporting, and record-keeping, a safety program, workforce adequacy and skill, and for quality control shall be devised by the Contractor, implemented as an element of services provided and ready for viewing by the Contracting Officer upon demand. The Government shall monitor the Contractor's performance using Quality Assurance procedures established for the contract. However, the Government reserves the right to utilize other methods as necessary to assure Contractor compliance with all terms and conditions of the contract. Additional costs to the Government caused by unsatisfactory work or nonperformance by the Contractor may be charged to the Contractor IAW, the FAR clause entitled quote mark Inspection of Service-Fixed-Price quote mark Section E (FAR 52.246-4). C.1.7. POST AWARD MEETING: Within 15 days of notice of award of the contract, the Contractor shall meet with the Directorate of Contracts and present the Contracting Officer with proof of adequate insurance, a list of employees who will be working on the contract and certifications that all employees can work on any piece of equipment as well as the unlined above in the contractors quality control assurance. C.1.8. Supplies, Materials, Replacement Parts, and Equipment The Contractor shall furnish all labor, supplies, equipment parts and materials necessary to perform cleaning, maintenance, inspection, repairs or replacements to elevator equipment, appurtenances, and accessories; including hoist machinery, motor generators, controllers, selectors, worm gears, thrusts, bearings, brake magnet coils, brake shoes, cab fans, brushes windings, commutators, rotating elements, contacts, coils, resistors for operation and motor circuits, magnet frames, cams, car door and hoisting door hangers, tracks and guides, door operating devices, interlocks and contacts, hatch lighting (minimum 60 watt illumination), pit lights, cab lights, bulb replacement in signal system, and all other elevator signal and accessory equipment complete, where included as part of the elevator installation at the time the offer for this work is submitted. The Contractor is responsible for replacement of all machine room light bulbs or tubes, hatchway and pit receptacles, ADA telephones equipment and light sockets. Particular attention is to be given to maintaining all emergency light units in an operable condition. All lighting installed or replaced by the Contractor shall be at a minimum compact fluorescent efficiency or greater. Where applicable, the Contractor shall obtain any special tools necessary to troubleshoot service and maintain the elevators related to the specific type of microprocessor controls installed on the system. This includes solid state or microprocessor diagnostic tools unavailable on the open market. All lubricants, oils, greases, rope preservatives, and cleaning materials are to be furnished by the Contractor and shall not be stored on site. Lubricants are to be of the proper grade for the intended use. All cleaners used to remove oil and grease shall be non-toxic, odorless and have a high flash point. The Contractor shall have replacement parts immediately available so that the elevator service will not be subject to interruptions and stoppages. C.1.9 Safety Inspection and Testing Service: Safety inspections of elevators required by the American National Standard Safety code for Elevators, Dumbwaiters and Handicap Lift will be performed under a separate inspection contract by a separate inspection contractor. The maintenance contractor will be notified of the schedule for these periodic inspections so that personnel shall be made available, at no additional cost, to accompany the contract inspector during the course of any inspection or test. The personnel provided by the maintenance contractor shall be qualified mechanics. The maintenance contractor's personnel shall participate in this inspection and testing process to the extent required by the inspection contractor. This participation requires, but is not limited to, operating the elevator and all controls, removing and replacing panel covers, supplying and safely moving the weights needed for car safety tests, and any other mechanical functions associated with the inspection and testing process. During the inspection and testing of elevators that takes place during normal working hours, at a minimum at least 50% of the passenger elevators hall remain in service at all times. 1. The contractor shall maintain elevators, equipment, appurtenances and accessories so as to comply with the requirements of the latest edition of ASME A17.1. 2. The equipment shall be inspected and tested in accordance with the applicable edition of ASME A17.1. The contractor shall confirm with the Contracting Officer's Representative (COR) when Semi-annual, Annual and Five Year Safety Inspections are usually scheduled. The contractor shall contact the COR a sufficient time in advance to arrange these inspections and remain in touch with COR and his/her inspector as necessary to complete the inspection process. 3. The Government reserves the right to inspect and test, as well as oversee inspections and tests, to ascertain that the above requirements are being fulfilled. C.1.10 Fire Protection and Safety: The Contractor shall maintain elevator equipment in compliance with the latest adopted edition of the Safety code for elevators and all other applicable codes and regulation. The Contractor shall promptly report to the COR or COR representative all known equipment deficiencies and provide prompt cost proposals for corrective work outside the scope of this contract, which may be required by the State Elevator Inspector, other code enforcement authorities or the contracting Officer. In situations where the KO, or COR need special advice, counsel, expertise, or interpretation in these areas to determine compliance with regulations, laws, standards and codes, the Fort Meade Fire Department may assist the KO or COR by providing the required expertise. While the responsibility for contract administration rests solely with the KO, the Fort Meade Fire Department shall be the sole authority for providing final interpretation of fire protection, life safety, occupational safety & health, and industrial hygiene regulations, standards, codes, laws and guidelines. C.1.11. Invoices: The Contractor shall submit original Invoices and must include service document / ticket, as necessary, signed by the COR or COR representative or a Government representative for approval prior to submittals to WAWF for payment. APPROVAL WILL BE DELAYED WITHOUT THIS REQUIRED SUPPORTING DOCUMENTATION OF SERVICES PERFORMED. Invoices must indicate the contract number, service performed (broken down by contract line item number), hours or rates expended to perform service, and costs to the Government. Payment approval will be delayed without the above stated information. C.2 DEFINITIONS AND ACRONYMS: The following definitions and descriptions apply whenever the work, phrase, or acronym is used in these specifications. C.2.1. Inspect: Visually assess the condition of the existing equipment both before and after service. C.2.2 Check: Make an appropriate assessment of the usefulness of the equipment once the service is complete. C.2.3. Replace: Remove equipment in whole or in part that the Government considers not fit for use. C.2.3. Record: Make notes concerning the functional status of the equipment as well as procedures taken (if needed) in making repairs. C.2.5. Call Back Service: The Contractor shall provide regular work hours and overtime call back service to correct equipment malfunctions. Call back service consist of responding by reporting to the building and beginning to take corrective action within two (2) hours of receiving request from the COR or other designated representative. Equipment malfunctions shall be corrected the same day that the service call was issued. If circumstances beyond the Contractor's control preclude resolution of the problem that day, the COR shall be notified in writing by email with an explanation of the delay. Under no circumstances shall the resolution take more than 24 hours after receipt of the call unless an extension is granted by the Contracting Officer or COR. The Contractor will be compensated for after hours call back service at the contracted hourly rate. Any invoice to the government for call back services shall not include travel time and costs, but rather the Contracted hourly rate. A. REGULAR Call Back Service - consist of responding to call back requests during the hours of 7:30 AM through 5:30 PM, Monday through Friday; Federal Holidays excluded. Regular call back Service will be provided at no additional cost to the Government. B. EMERGENCY Call Back Service - consist of promptly responding to requests for emergency Service 24 hours per day, 7 days per week. The Contractor shall report to the site of service for the emergency within 2 hour of the time of notification. If the situation cannot be resolved within Two (2) hour, the elevator shall be secured with the Contractor immediately notifying the DPW Work Control or the COR of the problem and the time and date corrective action will be completed. Request for service may be made by the COR or his/her representative by telephone. Emergency service consist of freeing individuals trapped in a stalled car, restoring inoperative elevators, or other situations determined by the COR to be an emergency nature. C.2.6 Document: Maintain (and be able to show) records of all delivery, replacement, and repair activities. C.2.7 Acceptable Quality Level (AQL): A level that represents the maximum percent of defective work, the maximum number of defects per hundred units of sampled work, or number of defects, that will be allowed before work is considered unsatisfactory. An AQL does not imply that the Contractor may knowingly perform in an unsatisfactory way. It means that the Government recognizes that unsatisfactory performance sometimes happens unintentionally. As long as unsatisfactory performance does not exceed the AQL, the service will not be subject to payment reduction by the Government. The Contractor, however, shall correct all unsatisfactory work. In addition to any inspection, complaints received by the government's Quality Assurance Evaluator (QAE)or authorized representative shall be used as supporting evidence to unsatisfactory performance documented by the QAE. (See the Performance Requirements Summary) C.2.8 Quality Control (QC): A method used by the Contractor to control the quality of goods and/or services produced by Contractor employees. C.2.9 Quality Assurance (QA): A method used by the Government to provide some measure of control over the quality of purchased goods and/or services received. The Government's QA Surveillance Program is not a substitute for Quality Control by the Contractor. C.2.10 Performance Requirements Summary (PRS): The performance requirements summary is a graphic summary of the Contractor's services or quote mark outputs quote mark and is used to measure the Contractor's performance utilizing and displaying such facts as: The degree of deviation from the work standard that the Government will accept without issuing penalties. How work will be inspected. The maximum percentage of deduction from the Contractor's award for unsatisfactory performance. C.2.11 Contractor: The term quote mark Contractor quote mark as used herein refers to both the prime Contractor and any subcontractors. The Contractor will be responsible for ensuring that his subcontractors comply with the provisions of this contract. C.2.12 Contracting Officer (KO): The person executing the resulting contract on behalf of the Government, and any other officer or civilian employee who is properly designated KO. C.2.13 Contracting Officer's Representative (COR): The COR is a Government employee selected and designated, in writing, by a KO to act as his authorized representative in administering a contract. The COR will evaluate the Contractor's performance within designated functional areas and perform other duties as authorized by the KO. C.2.14 Fed Spec: Federal Specification. C.2.15 Maximum Allowable Degree of Deviation (MADD) a term used as a more definitive substitute for AQL. C.3 GOVERNMENT FURNISHED PROPERTY: C.3.1 The Government will furnish no time, materials, equipment or services for this contract. C.4 CONTRACTOR FURNISHED PROPERTY: C.4.1 General C.4.1.1 Supplies, tools, services, vehicles and equipment required to perform this contract shall be provided by the Contractor at no additional cost to the Government. Vehicles and equipment shall meet applicable State and Federal Safety Regulations. C.4.1.2 The Contractor shall furnish all equipment and supplies required for operation and maintenance tasks outlined in Section C.5 at no additional cost to the Government. All materials shall be suitable for the services encountered and shall conform to the requirements of the equipment manufacturer on which the supplies will be used. C.4.1.3 The Contractor shall furnish cost estimates for government directed repair work when requested by the COR at no additional cost to the government. The cost estimate will include materials, labor, equipment cost, drawings and a short description of how he intends to complete the government directed repairs. C.4.1.4 The Contractor will be paid at the supplier's discounted price with not more than the percent markup for handling and overhead. All parts and material installed by the Contractor shall be shown on the Delivery Order and the invoice attached before payment will be authorized. Contractor shall provide evidence as to why existing components could not be repaired in lieu of replacement. All old parts that are replaced shall be retained by the Contractor for a period of 30 normal duty days for Government inspection. If determined by the Government to be salvageable, the Contractor shall turn the items over to the Contracting Officer. C.4.1.4.1 The Contractor shall provide three (3) copies of material submittals and supplier's cost for government directed repair work, to the Contracting Officer prior to purchasing any single part whose cost is in excess of $250.00. Material submittals shall include manufacturer's descriptive data in sufficient detail as to allow the Contracting Officer to evaluate the proposed product. Submittals shall include all information of the capacity, electrical characteristics, and pertinent operating data. The Government has the option to furnish any parts in excess of $250.00 in value. If the Government elects not to furnish any parts that cost more than $250.00, the Contractor shall furnish such parts. C.4.2 Facilities, Parts, Materials and Miscellaneous Supplies: C.4.2.1 Supply Requirements C.4.2.1.1 All Contractor-furnished supplies, parts and materials shall be new, of high quality, modern in design, and shall give long life and reliable operation. The items used shall be standard products of manufacturers regularly engaged in the production of such supplies, parts or materials. All parts and material shall be the same sizes, designs, and chemical composition, or better, as those replaced. The Contracting Officer serves the right to disallow the use of any Contractor proposed parts, materials or supplies which do not meet these requirements. C.4.2.1.2 The Contractor shall not use materials which do not fully comply with all applicable fire codes. If the Contractor needs to use or store any flammable material in any Government owned facility, written permission of the Contracting Officer shall be obtained first and safety procedures shall be in strict acceptance with all applicable fire regulations. C.4.2.2 Equipment used by the contractor to perform the work described herein. C.4.2.2.1 All equipment shall be designed and operated to prevent damage to government facilities. Damages sustained by the use of such equipment shall be repaired at the contractor's expense. C.4.2.2.2 Equipment judged by the Contracting Officer to be in a state of disrepair or otherwise not conforming to contact provisions shall be removed from the job. C.5 SPECIFIC TASKS AND STANDARDS: The Contractor shall perform all tasks identified in this section and shall perform its task to meet at least the minimum standards identified. C.5.1 Preventive Maintenance and Repair of Elevators, Dumbwaiters and Handicap Lift: C.5.1.1 Task, General C.5.1.1.1 The Contractor shall review and document the performance of all traction elevators and dumbwaiters and all hydraulic elevators to determine if any malfunctions have occurred with the cars since the last maintenance inspection was performed, and take corrective action (s) within the scope of Bid Item 0001. C.5.1.1.2 The Contractor shall check quantity and quality of all reservoirs, such as crankcases, gear boxes, buffers and oil cups. C.5.1.1.3 The Contractor shall ride all elevator cars to detect and repair any improper operation of car doors, hoist way doors, acceleration, leveling accuracy of floor stops, and the action of the machine brake. C.5.1.1.4 The Contractor shall test, and verify satisfactory operation of all elevator fire detection devices and emergency communication systems. C.5.1.1.5 The Contractor shall observe the operation of all relays, contactors and other devices for satisfactory operation and sequencing. Repair or correct the cause of any such excessive arcing and pitting. Perform incidental replacement of any contacts that show excessive arcing or pitting. C.5.1.1.6 The Contractor shall examine and adjust the brake plunger on all Elevators and Dumbwaiters. C.5.1.1.7 The Contractor shall wipe down and check for excessive heat, vibration and noses during operation and correct causes, of all Elevator, Dumbwaiter and Handicap Lift hoisting machines, hydraulic pumps and generators. C.5.1.1.8 The Contractor shall, with the main switch disconnected, operate each contractor and relay by hand. C.5.1.1.9The Contractor shall adjust and repair, slowdown and switches. C.5.1.1.10 The Contractor shall clean all hoist way door equipment. C.5.1.1.11 The Contractor shall check stile channels, car frame, cams, steadying plates and supports. C.5.1.1.12 The Contractor shall replace oil in gears, gear boxes crankcases, buffers, oil cups, bearings, hydraulic fluids and door operators with manufacturer's specified grades. C.5.1.1.13 The Contractor shall check, repair or perform incidental replacement of all button contracts, springs, wiring and landing lights. C.5.1.2 Standards, General C.5.1.2.1 All traction elevators, dumbwaiters, and handicap lift elevators are to be surveyed, twice monthly, findings recorded on the Elevator Deficiency Checklist (TE 5-002), or an equivalent form generated by the contractor. The results of the survey shall be submitted to the COR within three (3) working days of each elevator/dumbwaiter survey. Malfunctions replaced which had occurred between services were properly documented with special annotation and attention to any problem involving unsafe operations, provide photo of the damage part or parts and prepare a proposal for the repairs at market value plus discounts. C.5.1.2.2 Quarterly all Elevator, Dumbwaiter and Handicap Lift oil levels shall be maintained satisfactorily and replaced when dirty or contaminated, during the same maintenance period. C.5.1.2.3 Quarterly all Elevator, Dumbwaiter and Handicap Lift door interlocks, guides shoes, roller guides and retractable safety door edges shall be repaired, and examined for loose screws, bolts, worn pins, bearings, cams, rollers, and that the doors are securely locked so that the car does not stop because of an interrupted door circuit. Door interlock components shall be adjusted or incidental replacement as necessary for proper operation. C.5.1.2.4 Quarterly all Elevators Dumbwaiters and Handicap Lift equipped with fire fighters operation and special emergency service shall automatically or manually be tested to ensures that the elevator return to its designated level or floor as specified in ANSI A17.1 PHASE 1 OPERATION. C.5.1.2.5 C.5.1.2.6 Quarterly all Elevator, Dumbwaiter and Handicap Lift motors, generators, hydraulic pumps and hoisting machines shall be wiped down. Excessive heat, unusual noises and any oil leakage shall be corrected. Copper or carbon dust that accumulates between scheduled maintenance visits within or on control panels, equipment and elevator machine room spaces, including floors, walls and lighting fixtures, shall be removed. C.5.1.2.6 Semi- annually with the main line switch disconnected, operate each contactor and relay by hand. Adjust all control relays to ensure wiping action across the contacts. Replace all loose rivets and screws, adjust air gap if excessive, and ensure that there is no friction in the relay movement. Check hinge pins and stop plates, replacing those that are worn, check shunts, heavy wire terminals and overload relays for tightness of connections and check oil in overload relays, settings and operation of overloads. C.5.1.2.7 Semi-annually remove covers and clean out switch boxes, examine contacts, clean and adjust as required. Maintain all moving parts free from friction and noisy operation. Check the settings of Slow-down switches by operating the car into a terminal landing. If slow-down control is insufficient and the car goes past the terminal landing far enough to create a hazard to passengers in accordance with ANSI standards A17.1, then make corrective adjustments and or repairs. C.5.1.2.8 Semi-annually clean all grease and dirt from hanger tracks. Lubricate as recommended by the manufacturer. Check and adjust up thrust rollers. When applying fresh grease to the tracks ensure that the grease does not get into the door interlock bridge contracts. Examine the hanger fastenings to see that all parts are secure. Check the door gibes and replace those which are worn. Clean and lubricate the door arm, check for loose pins and bolts, worn and cracked castings and correct deficiencies found. C.5.1.2.9 Semi-annually check for bends, cracks and safety clearances on all stile channels, car frames, steadying plated and supports. Repair and adjust deficiencies found. The Contracting Officer shall be notified within 30 minutes of discovery when unsafe conditions cannot be corrected. C.5.1.2.10 Annually Lubricants shall be replaced, with manufacturer's specified grades in gears, gear boxes, crankcases, buffers, oil cups, bearings and door operators. All lubricants shall be disposed of in accordance with all State and Federal regulations. C.5.1.2.11 Annually check all button contacts, springs and wiring. Clean and lubricate as necessary. Repair or incidental replacement of defective parts shall be accomplished. Check landing lights and replace burned out lamps. C.5.1.2.12 Annually all Elevator, Dumbwaiter and Handicap Lift clearances between brake shoes and the drum shall be maintained at a minimum in accordance with manufacturer's recommendations. Brake contacts shall be examined for wear and adjusted or replaced. Brake drum pulleys shall be cleaned and loose material shall be prevented from catching between the drum and brake shoe. C.5.1.3 TRACTION Elevators and Dumbwaiters: (Tasks) C.5.1.3.1 Motor-generators, magnetic coils, relays and contractors shall be visually inspected devoting special attention to armatures, contacts, and relays that are continuously energized. Incidental replacement of coils that have brittle insulation or cracked binders shall be accomplished. The floor selector shall be inspected and checked, adjusted as necessary and lightly lubricated. C.5.1.3.2 The Contractor shall, with power removed, trip the governor by hand to see that all parts of the mechanism operate freely. C.5.1.3.3 The Contractor shall check and repair the retiring cam. C.5.1.3.4 The Contractor shall check all steel and iron ropes and fastenings on the car, counterweight and governor. C.5.1.3.5 The Contractor shall check and repair all traveling cable. C.5.1.3.6 The Contractor shall inspect and adjust the clearance of the counterweight and compensator at the buffer with the car stopped at the top floor landing. C.5.1.3.7 The Contractor shall inspect for and correct causes of cutting of drive sheave or secondary sheave groves. Lubricate all selector guides (sheaves), equalizers, guide rails, governor cable tension sheaves, oil buffers, regulators and small control motors, and terminal floor hangers. C.5.1.4 TRACTION Elevators and Dumbwaiters (Standards) C.5.1.4.1 Twice monthly check operation of all traction elevators' and dumbwaiters' motor-generators, control relays, contactors and other devices for proper sequence and physical condition. Where armatures, brushes and other electrical contacts are exposed to excessive current, circuits shall be checked, over currents corrected and burned contacts or burned or worn brushes shall be replaced. C.5.1.4.2 Monthly check for loose pins bolts and anchors. Examine and clean the over-speed safety switch contacts, check for proper operation and adjust if necessary. Lubricate the governor shaft and bearings and adjust or replace worn parts. C.5.1.4.3 Monthly check the retiring cam for worn pins and bearings, loose bolts and missing locking rings, incidental replacement shall be accomplished as required. Check the clearance between the cam and door switch lever rollers and adjust if necessary for proper operation. C.5.1.4.4 Monthly check all steel and iron ropes, fastenings on the drums, counterweights and governor. Ropes that have broken wires will require closer examination. Notify the Contracting Officer of adjustments made and of unsafe conditions found before leaving the job site, on the day of this inspection. A determination to replace any wire rope will be made by the Contracting Officer. The tension of hoist and compensating ropes shall be checked and adjusted if necessary to ensure that the tension is equal. C.5.1.4.5 Monthly checks and repairs all traveling cables to see if there is evidence of wear due to rubbing against any operating beams walls or the car platform. All bare spots shall be covered with electrical tape to prevent further damage. Adjust the close loop to avoid striking the pit floor and ensure that the line travels through the hoist way without any twisting motion or interference. C.5.1.4.6 Monthly inspect and adjust the clearance of the counterweight at the buffer with the car stopped at the top floor landing in accordance with manufacture's recommendations. Check the clearance between the compensator and pit floor to determine if the ropes on either the counterweight or on the compensator need to be shortened. Examine and adjust the compensating sheave and sheave bearings. Lubricate sheave bearings. C.5.1.4.7 Monthly inspects and repairs the drive sheave or secondary sheave if cutting is evident. The hoisting ropes shall be lightly lubricated and other corrective action taken. Maintain the required oil level in the gear housing. Check the gear backlash, thrust end play, and bearing wear. Repair all oil leaks. C.5.1.5 HYDRAULIC Elevators and Dumbwaiters: (Task) C.5.1.5.1 Magnetic coils, control relays and contactors shall be visually inspected devoting special attention to coil armatures contacts, and relays that are continuously energized. Incidental replacement of coils that have brittle insulation or cracked binders shall be accomplished. The floor selector shall be inspected and checked, adjusted as necessary and lightly lubricated. C.5.1.5.2The Contractor shall check and repair cylinder packing glands. C.5.1.5.3The Contractor shall empty all oil collection containers. C.5.1.5.4The Contractor shall check and repair, pumps and system piping. C.5.1.5.5 The Contractor shall maintain proper hydraulic oil levels in holding tanks. C.5.1.5.6HYDRAULIC Elevators and Dumbwaiters: (Standards) C.5.1.6.1 Quarterly check operation of all hydraulic elevators' magnetic coils, control relays, contractors and other devices for proper sequence and physical condition. Where electrical contacts are exposed to excessive current, circuits shall be checked, over currents corrected and burned contacts shall be replaced. C.5.1.6.2 Quarterly checks and tightens or replaces cylinder packing glands when leakage is evident. C.5.1.6.3 Quarterly empty the oil collection container during each maintenance inspection to prevent oil from leaking into the pit. C.5.1.6.4 Quarterly wipes down the pump and motor. Check all bearing surfaces for excessive heat and unusual noises while the hydraulic pump unit is running, and adjust and repair to correct any deficiencies found. Any oil leakage that develops at the pump and or piping system shall be corrected. C.5.1.6.5 Quarterly run the car up and down and observes the oil level at each extreme of the car. Add hydraulic oil if necessary to ensure satisfactory levels. Replacement oil shall comply with equipment manufacturer's specifications. C.5.1.7 Routine Inspections and Tests: (Task) C.5.1.7.1 Contractor shall perform routine inspections and tests, to be witnessed by a Certified Third Party Elevator Inspector. Contractor shall notify the Contracting Officer (14) fourteen days prior to the scheduled inspection and test dates. C.5.1.8 Routine Inspections and Tests: (Standards) C.5.1.8.1Semi-annually perform routine inspections and tests in accordance with ANSI A17.1a, Section 1001. C.5.1.9 Periodic Inspections and Tests: (Task) C.5.1.9.1 Contractor shall perform periodic inspections and test, to be witnessed by Certified Third Party Elevator Inspector. Contractor shall notify the Contracting Officer (14) fourteen days prior to the scheduled inspection and tests dates. C.5.1.10 Periodic Inspections and Tests: (Standards) C.5.1.10.1Annually perform periodic inspections and tests in accordance with ANSI A17.1A, Section 1002. C.5.2 Inspect: C.5.2.1 Task: The Contractor shall inspect all existing equipment and parts included in the provisions of this contract for signs of damage or wear which could negatively affect operation. Materials brought to the site shall be in their original boxes with unbroken seals. The materials shall be examined at the site by the Contractor for damage prior to installation. All parts brought to the site that do not adequately renovate or repair the existing equipment shall be removed and replaced with adequately working parts at no cost to the Government. C.5.2.2 Standard: An accurate assessment of the condition of all existing equipment will be made. C.5.3 Operation: C.5.3.1 Task: The Contractor shall check the operation of equipment after renovations or repairs for normal functioning prior to leaving the site. C.5.3.2 Standard: An accurate assessment of the normalcy of operation of equipment will be made such that disruption in service of the equipment will be minimized. C.5.4 Repair-Maintenance: C.5.4.1 Task: The Contractor shall repair inoperative or malfunctioning equipment to ensure that the existing equipment is in working order prior to leaving the site as required under the terms of the contract. The Contractor shall maintain the equipment to minimize the time when equipment is inoperative. C.5.4.2 Standard: Repair efforts will restore equipment to proper operation. C.5.5 Service-Maintenance: C.5.5.1 Task: The Contractor shall maintain all designated elevators, and dumbwaiters contractually targeted herein for servicing such as to prevent its breakdown and malfunction, i.e., in order to prolong the functional life of such equipment. C.5.5.1.1 Maintenance shall be performed as recommended by the manufacturer for the Elevator, Dumbwaiter and Handicap Lift equipment for optimum operation. C.5.5.1.2 The Contractor will replace equipment or parts whose normal function cannot be restored through repair procedures. C.5.5.1.3 The Contractor may be required to renovate existing equipment when the manufacturer of the existing equipment is no longer in business and the existing equipment cannot be economically or practically replaced. C.5.5.2 Standard: The life of Elevator, Dumbwaiter and Handicap Lift equipment will be prolonged within the limits produced by normal wear; and in situations where time, normal wear, and/or damage has compromised equipment integrity, replacement of equipment will restore all functions previously carried out by the irreparable machinery or parts. C.5.6 Record and Document: C.5.6.1 Task: The Contractor shall record all delivery activities as specified in the work contract and maintain documents of the same. C.5.6.2 Standard: Records will exist and be available for inspection at all sites and locations specified by QAIs. C.5.7 Follow Required Schedules: C.5.7.1 Task: The Contractor shall make repair or replace elevator or dumbwaiter equipment on an as-needed basis, C.5.8 Inspection and Testing: C.5.8.1 Task: The Contractor shall inspect and test each Elevator, Dumbwaiter and Handicap Lift on an annual basis for yearly certification. The Contractor shall inspect and test the elevators and dumbwaiters selected by the COR for the five (5) year certification. The Contractor shall supply all the equipment, materials and labor necessary for the inspections and tests. C.5.8.2 Standard: Certification of the tests will be by another independent Government Contractor. The task will not complete until each Elevator, Dumbwaiter and Handicap Lift is certified. C.5.8.3. REQUIRED INSURANCE a. The Contractor shall be required to carry the following minimum insurance during the entire period of his performance under this contract: TYPE AMOUNT (1) Worker's Compensation and Employer's Liability$100,000 (2) Comprehensive General Liability Bodily Injury Per Occurrence $500,000 (3) Comprehensive Automobile Liability Bodily: Injury Per Person$200,000 Bodily Injury Per Occurrence $500,000 Property Damage Per Occurrence $ 20,000 b. Prior to the commencement of work hereunder, the Contractor shall furnish to the Contracting Officer a certificate or written statement of the above required insurance. The policies evidencing required insurance shall contain an endorsement to the effect that cancellation or any material change in the policies shall not be effective for such periods as may be prescribed by the laws of the state in which this contract is to be performed and in no event less than thirty (30) days after written notice thereof to the Contracting Officer. c. The Contractor agrees to insert the substance of this clause including this paragraph, in all subcontracts hereunder that require work on a Government installation and shall require subcontractors to provide and maintain the insurance required in the schedule or elsewhere in the contract. The Contractor shall maintain a copy of all subcontractors' proofs of required insurance, and shall make copies available to the Contracting Officer upon request. C.6. APPLICABLE PUBLICATIONS: C.6.1Contractor shall perform inspections, maintenance and repairs required by the sections of C-5 (Tasks and Standards). Contractor shall perform all work in accordance with the standards outlined in the individual sections as well as the following publications. The latest version of each standard will be enforced. C.6.2 American National Standards Institute, Inc. ANSI/ASME A17.1 Safety Code for Elevators and Escalators ANSI A17.2 American Standard Practice for the Inspection Of Elevators, Escalators and Moving Walks, Inspectors Manual C.6.3 Industry Standards: Building Officials and Code Administrators (BOCA) Codes National Electrical Code National Plumbing Code National Fire Protection Association Standards (NFPA) Equipment Manufacturers Literature and Recommended Maintenance Practices C.7 INSTALLATION ACCESS CONTROL Credential Standards a. OMB M-05-24 - Contractors presence at Fort George G. Meade (FGGM) for six months or more. All contractor employees must meet fitness standards in accordance with Homeland Security Presidential Directive (HSPD) 12 and Enclosures, and FGGM Policy Letter 70 - Access Control for issuance of an Identification Credential. The Government reserves the right to make changes on other security references as they come about during the life of this contract. To meet these standards the investigations will be requested by the contractor via the Personnel Security Investigation Portal (PSIP) worksheet and submitted to the Government Security Office NLT 10 working days PRIOR to a potential employee's startup performance date. The Government Security Office will provide status within 10 working days from receipt of the PSIP. The Contractor's employees must read, write and speak English. Contractor's Project Manager has five (5) calendar days to ensure that the potential employee's security documentation is completed. Incomplete security documentation will not be accepted by the Government and the request for investigation will be subject to termination. b. AR 190-13, Army Physical Security Program- Contractors presence at Fort George G. Meade (FGGM) for six months or less, requires all Contractors to successfully undergo a background check through the National Crime Information Center (NCIC) Interstate Identification Index (III), which is the Army minimum baseline background check for entrance onto Army installations for non-CAC holders to ensure the security of military installations. C.7.1 Reporting of Security Incidents In accordance with Homeland Security Presidential Directive (HSPD) 12, Background, Enclosure 1, quote mark If at any time during employment or service, an Army Soldier, civilian or contractor is found to not meet HSPD-12 credentialing standards, or determination is made that the individual's possession of a CAC poses an unacceptable risk as defined in reference 7, the individual's CAC may be revoked. If the Contractor is aware of any adverse information regarding their personnel during the life of the contract, the Contractor is responsible to report it within 24 hours to the Contracting Officer. Executive Order 13488 of January 16, 2009 - Definitions: quote mark Fitness quote mark is the level of character and conduct determined necessary for an individual to perform work for or on behalf of a Federal agency as an employee in the excepted service (other than a position subject to suitability) or as a contractor employee. quote mark Fitness determination quote mark means a decision by an agency that an individual has or does not have the required level of character and conduct necessary to perform work for or on behalf of a Federal agency as an employee in the excepted service (other than a position subject to suitability) or as a contractor employee. A favorable fitness determination is not a decision to appoint or contract with an individual. BACKGROUND CHECKS PROHIBITION ON USE OF CONTRACTOR PERSONNEL WITH NEGATIVE CRIMINAL HISTORY SCREENINGS The successful offeror agrees to comply with all installation access control procedures and policies prior to and during the performance of this contract. Successful contractor shall submit a complete list of all employees it intends to use for government work for a criminal history background check. This list shall contain the full name of each employee, his/her social security number, or, if a resident alien, his/her U.S. work visa number, date of birth, social security card and dates the employee is expected to work on the installation. This requirement applies equally to successful offerors and any subcontractors the successful offeror may use. Government may, in its sole discretion, conduct a criminal history check on each successful offeror or subcontractor employee for the purpose of screening the potential security risk each employee may present as he/she works on the installation. The government shall determine the risk each employee presents to the installation, and may, in its sole discretion, prevent individual employees from gaining access to this installation. The government shall notify the successful offeror in writing as to the acceptability of each employee, however, the government reserves the right to reject any employee at a later date. If the government determines that the employee is unacceptable, the government shall provide the offeror a written notice with the name of the employee denied access. Offeror shall be responsible for finding a suitable replacement worker who may be granted temporary permission to work under the contract until a criminal history check has been completed. If the successful contractor is unable to replace the employees, the government reserves the right to determine the bid as non-responsive and deny award of the contract to the offeror. If, after award of the contract and during the course of performance the successful offeror now as contractor finds it necessary to substitute employees under the contract, contractor shall submit the same information of those replacement employees as stated above. Successful offeror as contractor agrees to update the list for each new employee hired and working under this contract. If the government denies the employee access, and the contractor is unable to find suitable replacements, the government may consider other available remedies, such as: 1. Terminate the contract for the convenience of the government at no cost to the government: or 2. Termination of the contract for Cause C.7.2 Successful offeror/contractor understands that the installation access control procedures may delay the time it takes to enter or exit the installation and may prevent the contractor from working totally on certain days or periods of time due to increased threat condition levels. Successful offeror/contractor assumes the risk of such delays and agrees that such delays will not be considered as a basis of claim against the government and agrees to waive any and all claims or causes of action that may arise under law as a result of such delays. The successful offer or agrees to comply with all installation access control procedures and policies prior to and during the performance of this contract. Successful contractor shall submit a complete list of all employees it intends to use for government work for a criminal history background check. This list shall contain the full name of each employee, his/her social security number, or, if a resident alien, his/her U.S. work visa number, date of birth, and dates the employee is expected to work on the installation. This requirement applies equally to successful offeror and any subcontractors the successful offeror may use. C.7.3 Vehicle Registration: All individuals name should also include the requirement that all vehicles used by the contractor's employees will be properly registered prior to providing access to the post. Failure of the individual to have a proper background check and vehicle registration will not be cause of claim against the government if it precludes the contractor's personnel from performing under the contract.
