Vendor Questionnaire W-9 Bread Supplier - Scope of Work Scope of Work: Provide weekly service for fresh bread items in our Job Corps cafeteria Include product delivery of: 20oz white wheat sandwich bread; 20oz wheat sandwich bread; 32 ct dinner rolls; 16 ct hotdog buns; 12 ct hamburger buns; 12 ct Texas Toast; 20 oz white sandwich bread; 6 pk hoagie rolls; Include Purchase Order on each invoice and remove empty trays every week by vendor. Include W-9; Vendor Questionnaire along with bid (attached) If HubZone or disadvantaged (8a) business; please include a copy of your certification.
