Description
INTRODUCTION This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The solicitation number is HHS-NIH-NIDA-CSS-2013-3027550 and the solicitation is issued as a request for quotation (RFQ). Point of Contact: John Flannery, Contracting Officer, Telephone: (301)-594-2490, Email: John.Flannery@nih.gov ACQUISITION AUTHORITY This acquisition is for a commercial item or service and is conducted under the authority of the Federal Acquisition Regulation (FAR) Part 13-Simplified Acquisition Procedures; FAR Subpart 13.5-Test Program for Certain Commercial Items; and FAR Part 12-Acquisition of Commercial Items, and is not expected to exceed the simplified acquisition threshold. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-69, dated August 1, 2013. The provision at FAR clause 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition. Offerors must include a completed copy of the provision at FAR clause 52.212-3, Offeror Representations and Certifications - Commercial Items, with its offer. Contract Terms and Conditions - Commercial Items per FAR clause at 52.212-4 apply to this acquisition. Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, found at FAR clause at 52.212-5, also apply to this acquisition. The Defense Priorities and Allocations System (DPAS) are not applicable to this requirement. SMALL BUSINESS SET ASIDE STATUS This combined synopsis/solicitation is set aside for small businesses. The associated NAICS code is 621511 and the small business size standard $30M. BACKGROUND INFORMATION & PROJECT REQUIREMENTS: The National Institute on Aging (NIA), Intramural Research Program (IRP), has a requirement for soft tissue for use in the NIA IRP laboratories in Baltimore, Maryland. The NIA IRP requires the following items and quantities: 1. Soft tissue, unstained, on 1x3" slide. Quantity: 8,740. 2. H&E stained soft tissue on 1x3" slide. Quantity: 437. 3. Special trichrome staining. Quantity: 437. 4. Slide box, 100 capacity. Quantity: 97. The contractor shall deliver these items within 60 days after receipt of order to the following address: The National Institute on Aging 251 Bayview Blvd. Baltimore, MD 21224 EVALUATION CRITERIA The Government intends to award a purchase order resulting from this solicitation to the responsible offeror whose offer is lowest price and technically acceptable. A firm fixed price purchase order is anticipated. RESPONSE REQUIREMENTS Interested suppliers are encouraged to submit a quotation responding to the listed requirements. Responses to this notice shall contain sufficient information to establish the interested parties' bona-fide capabilities for fulfilling the requirement and must include: 1. Unit Price 2. List Price 3. Shipping and Handling Costs (Quoted as a separate line item) 4. The delivery period after contract award 5. Any prompt payment discount terms 6. Contractor Dun & Bradstreet Number (DUNS) 7. Contractor Taxpayer Identification Number (TIN) 8. Contractor Point of Contact Information All offerors must have an active registration in the System for Award Management (SAM) www.sam.gov. Small business certification must be included in the contractor's SAM profile if the contractor is quoting as a small business. All responses must be received by September 27, 2013 at 9:00 AM EST and must reference solicitation number HHS-NIH-NIDA-CSS-2013-3027550.
