As used in this provision-- Data Universal Numbering System (DUNS) number means the 9-digit number assigned by Dun and Bradstreet, Inc. (D&B) to identify unique business entities. Data Universal Numbering System +4 (DUNS+4) number means the DUNS number assigned by D&B plus a 4-character suffix that may be assigned by a business concern. (D&B has no affiliation with this 4-character suffix.) This 4-character suffix may be assigned at the discretion of the business concern to establish additional System for Award Management records for identifying alternative Electronic Funds Transfer (EFT) accounts (see the FAR at Subpart 32.11) for the same parent concern. Registered in the System for Award Management SAM database means that-- (1) The offeror has entered all mandatory information, including the DUNS number or the DUNS+4 number, the Contractor and Government Entity (CAGE) code, as well as data required by the Federal Funding Accountability and Transparency Act of 2006 (see Subpart 4.14) into the SAM database; (2) The offeror has completed the Core, Assertions, and Representations and Certifications, and Points of Contact sections of the registration in the SAM database; (3) The Government has validated all mandatory data fields, to include validation of the Taxpayer Identification Number (TIN) with the Internal Revenue Service (IRS). The offeror will be required to provide consent for TIN validation to the Government as a part of the SAM registration process; and (4) The Government has marked the record ''Active''. (b)(1) By submission of an offer, the offeror acknowledges the requirement that a prospective awardee shall be registered in the SAM database prior to award, during performance, and through final payment of any contract, basic agreement, basic ordering agreement, or blanket purchasing agreement resulting from this solicitation. (2) The offeror shall enter, in the block with its name and address on the cover page of its offer, the annotation "DUNS" or "DUNS +4" followed by the DUNS or DUNS +4 number that identifies the offeror's name and address exactly as stated in the offer. The DUNS number will be used by the Contracting Officer to verify that the offeror is registered in the SAM database. (c) If the offeror does not have a DUNS number, it should contact Dun and Bradstreet directly to obtain one. (1) An offeror may obtain a DUNS number-- (i) Via the Internet at http://fedgov.dnb.com/webform or if the offeror does not have internet access, it may call Dun and Bradstreet at 1-866-705-5711 if located within the United States; or (ii) If located outside the United States, by contacting the local Dun and Bradstreet office. The offeror should indicate that it is an offeror for a U.S. Government contract when contacting the local Dun and Bradstreet office. (2) The offeror should be prepared to provide the following information: (i) Company legal business. (ii) Tradestyle, doing business, or other name by which your entity is commonly recognized. (iii) Company Physical Street Address, City, State, and Zip Code. (iv) Company Mailing Address, City, State and Zip Code (if separate from physical). (v) Company Telephone Number. (vi) Date the company was started. (vii) Number of employees at your location. (viii) Chief executive officer/key manager. (ix) Line of business (industry). (d) If the Offeror does not become registered in the SAM database in the time prescribed by the Contracting Officer, the Contracting Officer will proceed to award to the next otherwise successful registered Offeror. (1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract. (2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved. (3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law. The Contractor will be required to provide consent for TIN validation to the Government as part of the SAM registration process; and (4) The Government has marked the record "Active." (End of provision) 252.209-7995 REPRESENTATION BY CORPORATIONS REGARDING AN UNPAID DELINQUENT TAXLIABILITY OR A FELONY CONVICTION UNDER ANY FEDERAL LAW-FISCAL YEAR 2013 APPROPRIATIONS (DEVIATION 2013-O0010) (APR 2013) (a) In accordance with sections 8112 and 8113 of Division C and sections 514 and 515 of Division E of the Consolidated and Further Continuing Appropriations Act, 2013,(Pub. Whenever Contractor personnel are required to perform work aboard a ship of U.S. Navy or associated Shore Facility, the Contractor shall submit a written request to visit that ship/facility to the facility or ship Security Officer and to Code @@. (1) For classified visits, the visit request shall include all information required by Chapter 6 of DoD 5220.22-M "National Industrial Security Program Operating Manual (NISPOM)". (2) For unclassified visits, the visit request shall include the following information on each person visiting the user agency's facility or ship. No employee or representative of the Contractor shall be admitted to any facility or ship of the U.S. Navy unless satisfactory proof of citizenship of the United States has been furnished. c. Access Badge. Each contractor/subcontractor employee must obtain an access badge prior to visiting or performing work on a Government installation. Two pieces of identification and a completed copy of the Department of Homeland Security Form I-9 "Employment Eligibility Verification" are required prior to issuance or reissuance of an access badge. Form I-9 is available at the following website: http://www.uscis.gov/graphics/formsfee/forms/i-9.htm. d. Compliance with Security Regulations. (1) The Contractor shall comply with the security regulations in force at @@. Company-granted clearances are not valid for access to restricted areas or information. If access to these areas or information is involved, contractor personnel must possess a Government-granted clearance based on formal investigative actions. Contractor personnel may be required to fill out a form regarding involvement with alcohol or dangerous drugs, police records, relatives living abroad, and personal foreign travel as a condition for receiving a clearance. (2) If applicable, special security provisions for access to classified information are set forth in the attached DD Form 254, Contract Security Classification Specifications. (3) It is the Contractor's responsibility to collect and account for all personnel identification passes/badges and vehicle passes issued to his employees when no longer needed. The Contractor must return all passes/badges to the facility security office within three (3) days of the separation of an individual from employment on this contract. (4) It is the Contractor's responsibility to assure that his employees are aware of, and comply with, the security requirements of the facility or ship as set forth in the contract and as explained at the pre-award or post-award conference, if applicable. Noncompliance by an individual can result in denial of access to the facility or ship. (5) If a Contracting Officer's Representative (COR) is appointed in this contract, a listing of persons employed on the contract indicating that they have been briefed on the facility/ship security requirements shall be submitted to the COR within 14 days after award of the contract. The list is in addition to that shown in paragraphs a, b, and c above. The list shall also include a description of all company vehicles, including road equipment and office supply trailers, to be employed on the facility, along with license numbers, if applicable. The Contractor shall promptly notify the COR of any changes to the list. INSPECTION AND ACCEPTANCE (DESTINATION) Inspection and acceptance of the supplies or services to be furnished hereunder shall be made at destination by the receiving activity. SURVEILLANCE OF FIRM-FIXED-PRICED SERVICES a. The official(s) designated below shall be responsible for appropriate surveillance of all services to be performed under this contract. When or if, in the opinion of the contractor, an effort outside of the existing scope of the contract is requested, the Contractor shall promptly notify the Administrative Contracting Officer (ACO) in writing. No action shall be taken by the contractor under such direction until the Contracting Officer or ACO has issued a modification to the contract or has otherwise resolved the issue. PLACE OF PERFORMANCE (SERVICES) Services performed hereunder by the Contractor shall be performed at: NAS WHIDBEY ISLAND & NAVAL STATION EVERETT REVIEW OF AGENCY PROTESTS (a) The procedures set forth in FAR 33.103 will be followed for submission and handling of agency protests. (b) All protests filed directly with the agency, NAVSUP Fleet Logistics Center Puget Sound (FLCPS), shall be addressed to the Contracting Officer. A protester may appeal a decision rendered by a Contracting Officer to the Reviewing Official. (c) The Reviewing Official for FLCPS is the Director, Regional Contracting Department (Code 200), NAVSUP Fleet Logistics Center Puget Sound at 467 W Street, Bremerton, WA 98314-5100. (d) Offerors should note that a review of the Contracting Officer's decision will not extend GAO's timeliness requirements. Therefore, any subsequent protest to GAO must be filed within 10 days of knowledge of initial adverse agency action. USE OF PAST PERFORMANCE INFORMATION RETRIEVAL SYSTEM - STATISTICAL REPORTING (PPIRS-SR) IN PAST PERFORMANCE EVALUATION (1) The offeror's past performance shall be evaluated in accordance with FAR 13.106-2 or 15.305(a)(2), as applicable. This procurement is subject to Past Performance Information Retrieval System - Statistical Reporting (PPIRS-SR), authorized by the Department of Defense for use by participating activities during the acquisition of supplies and services. (2) The purpose of the PPIRS-SR is to provide contracting personnel with quantifiable past performance information regarding delivery and quality, as collected from existing DoD reporting systems. PPIRS-SR uses the past performance data collected to classify each supplier's performance by Federal Supply Class(es) or Service Code(s). (3) Quality : The method used in PPIRS-SR to classify the quality of a supplier's past performance is a comparison of quality records among all suppliers who have provided goods or services to the Federal Government which are categorized in the same Federal Supply Class or Service Class (FSC). Based on the comparison among suppliers in a specific FSC group, suppliers are classified by PPIRS-SR into a color rating that represents the supplier's overall quality performance for that specific Federal Supply Class or Service Class. The following is a summarization of the PPIRS-SR classification colors: 1. Dark Blue: Top 5% of suppliers in FSC group* 2. Purple: Next 10% of suppliers in FSC group* 3. Green: Next 70% of suppliers in FSC group* 4. Yellow: Next 10% of suppliers in FSC group* 5. Red: Bottom 5% of suppliers in FSC group* * If all supplier performance ratings for an entire FSC group are equal, all suppliers in that group will be classified Green. (4) Delivery : Supplier delivery past performance is classified in PPIRS-SR by the supplier's percentage of on time deliveries. STATEMENT OF WORK PERFORMANCE WORK STATEMENT FOR ROMAN CATHOLIC (RC) PRIEST SERVICES AS CONTRACT RELIGIOUS MINISTRY PROFESSIONAL (CRMP) IN SUPPORT OF THE COMMAND RELIGIOUS PROGRAM (CRP) OF NAVAL AIR STATION WHIDBEY ISLAND (NASWI) AND NAVAL STATION EVERETT (NSE) 1.0 BACKGROUND. Per US Navy Regulations, SECNAVINST 1730.7D, and OPNAVINST 1730.1E it is incumbent upon commanding officers (COs) to have a CRP which accommodates the religious needs, preferences, and rights of assigned military personnel, eligible family members, and other authorized users. Per SECNAVINST 5351.1, at installations where the assigned chaplains cannot provide adequately for the variety of represented faith groups, it is most appropriate to augment the CRP with CRMPs to help execute mission requirements. Contracting the services of a civilian RC priest as a CRMP is authorized and appropriate. 1.1. The object of this contract is to secure the offices of a Roman Catholic priest to provide faith-specific religious ministry to authorized RC personnel, leadership and oversight to the RC portion of the CRP, and advice to the Command Chaplain. The contractor shall perform services at the Chaplain's office spaces, chapel, and other places as determined by the NASWI Command Chaplain. 2.0. PERFORMANCE. The performance period for this contract is 01 October 2013 - 30 September 2014. The CRMP shall: 2.1. Report to the NASWI Command Chaplain 2.2. Provide for RC worship, sacramental ministrations, religious guidance, scripture study sessions, and religious education. Provide RC pastoral care/counseling to authorized personnel as requested, required, and scheduled. 2.3. Assist the Command Chaplain with advice in defining, validating, and implementing CRP Planned Ministry Objectives (PMOs) for RC personnel based on religious requirements and by keeping the Command Chaplain informed on all pertinent issues/concerns/matters. 2.4. Demonstrate care for all CRP customers and facilitate the religious needs of non-RC personnel by referring them to other members of the CRP. 2.5. Accomplish administrative requirements in a timely fashion. 2.6. Be neat, clean, well-groomed, and in appropriate clergy clothing when in public areas. 2.7. Become acquainted with and obey all pertinent Command, DON, and DOD instructions; shall perform in a manner which precludes any waste of government resources; and shall not use government equipment for personal business. All motor vehicles operated on the installation by the contractor shall be registered with base security and operated according to applicable directives. 2.8. Although in fact not a Government employee, shall comply with Executive Order 11222 of May 8, 1965, Prescribing Standards of Ethical Conduct for Government Officers and Employees. 3.0 TASKS. Execute the CRP PMOs for RC personnel. 3.1. Provide Roman Catholic Mass and Reconciliation (confession). 3.1.1. Conduct Catholic Mass every Saturday (NSE) and Sunday (NASWI) as scheduled. 3.1.2. Be on site and available for confessions each week for one hour prior to the start of Mass and for fellowship opportunities for one half hour after Mass. 3.1.3. Conduct one weekday Catholic Mass as scheduled at NASWI. 3.1.4. Conduct Catholic Mass on all Holy Days of Obligation at NASWI and NSE. 3.1.5. Conduct Advent and Lenten Penance Services at NASWI. 3.1.6. Conduct annual First Communion and Confirmation Services at NASWI and NSE. 3.1.7. Conduct all other Mass requirements as defined in the CRP's Planned Ministry Objectives (PMO). Other worship services will be performed as needed and at the discretion of the Command Chaplain. 3.1.8. Conduct and effectively oversee the training and certification of worship volunteers as required. 3.1.9. Compose Prayers of the Faithful for Sunday and Holy Day liturgies with respect to liturgical guidelines and the needs of the CRP. 3.2. Perform additional sacramental and emergency ministrations. 3.2.1. Prepare personnel and authorized family members for sacramental ministrations. 3.2.2. Schedule, administer, and conduct Baptism, First Communion, Confirmation, and Marriage Services. 3.2.3. Provide Anointing of the Sick and Funeral/Memorial Services as requested and authorized. 3.3. Conduct and supervise RC religious education and spiritual formation programs at NASWI and NSE. 3.3.1. Instruct and supervise the Rite of Christian Initiation of Adults (RCIA), scripture study sessions, and the Confraternity of Christian Doctrine (CCD) programs at NASWI and NSE. 3.3.2. As DRE, o versee the Catholic Religious Education Program, providing guidance to the Catholic Coordinator for Religious Education (C-CRE) as required (whether contractor or volunteer) and volunteer teachers within the RC education program as required. 3.4. Provide pastoral care/counseling and spiritual support. 3.4.1. Provide individual and/or group counseling sessions for spiritual formation, marriage preparation, baptismal preparation, and personal and/or family matters. 3.4.2. Attend community-building fellowship events. 3.5.1. Provide timely information and professional guidance to CRP personnel for worship service programs/bulletins, required sacramental documentation, and other various administrative requirements. 3.5.2. Ensure the timely submission of all required contract documentation. 3.5.3. Preside over the Catholic Chapel Council and forward the Council's meeting minutes and recommendations to the Command Chaplain for consideration. 3.5.4. Maintain ministerial faculties and liaison with the Archdiocese for the Military Services, USA (AMS). This includes advising AMS of any change in ecclesiastical status or contact information along with personal attendance at the annual priest convocation convened by AMS. All costs associated with maintaining professional credentials will be borne by the contractor. 3.5.5. Perform various administrative tasks as required. This includes quarterly reports and other such documentation as may be required by AMS. The command chaplain will be provided a copy of this report. 3.5.6. Complete necessary training and maintain currency on the use of such government office equipment and computers as required. 3.5.7. Maintain current certification for access to the installation. Comply with station and government policies and regulations in regard to identification badges, parking, and professional decorum. 3.5.8. Provide Lessons Learned to the Command Chaplain within 5 days of any problems encountered and resolved. 3.5.9. Direct concerns regarding morale, breach of ethical standards, or moral infractions to the Command Chaplain as soon as possible. 3.5.10. Notify the Command Chaplain, in advance, when unable to perform services due to scheduled time off, illness, inclement weather and/or emergency leave. 3.5.11. Arrange for scheduled absences at least two weeks in advance. 3.5.12. In advance of scheduled absences, secure the services of a credentialed substitute from a list approved by the AMS and previously provided to the Command Chaplain. Exceptions are permitted for a local priest, who is not AMS-approved, to fill in for the CRMP as long as he has a letter from his bishop/superior granting that exception for a specific occasion or period of time. 3.5.13. Provide and compensate the pre-approved substitute when unable to perform services required within this statement of work. 4.0 DELIVERY SCHEDULE. Offerors for this contract must be ordained Roman Catholic priests with the requisite skills and experience acceptable to AMS. Personnel proposed for this contract must be able to qualify for a Confidential Security Clearance at a minimum. 6.0 PLACE OF PERFORMANCE: Naval Air Station Whidbey Island Chapel and Naval Station Everett Chaplain Office (Bldg. 2000) 7.0 ADMINISTRATIVE REQUIREMENTS 7.1 Computer Access · Access to NMCI is required. The contractor must complete Information Assurance training; submit a SARS-N form; and have completed the appropriate background investigation necessary to obtain an NMCI account and a CAC. The government does not anticipate work under this task order will require access to classified information. 7.2 Government Furnished Property and Information: · The Government activity will provide adequate work space on NASWI and NSE and all necessary equipment (e.g. desk, chair, computer, printer access, copier access and telephone) for the duration of this task order. The contractor shall maintain the assigned office space in a neat and orderly manner. The contractor shall only use Government furnished resources in support of work under this contract. Personal or company use of telephones, utilities, computers, printers, copiers, etc. not directly related to required services is strictly prohibited. 7.3 Training · The Government will not reimburse costs associated with contractor training in an effort to attain and/or maintain minimum personnel qualification requirements of this contract. Attendance at workshops or symposiums is considered training. Contractor may attend government specific training as required. 7.4 Contractors in the Government Workplace Contractor employees are required to identify themselves as follows: · All Contractor spaces/desks on a government site shall be clearly marked as contactor spaces. · All email sent by Contractor personnel from a government computer shall identify the company name in the signature block. · All Contractor correspondence sent to the government shall be clearly marked with company name or letterhead. · All Contractor personnel shall insure that they identify themselves as Contractor personnel when answering a government telephone, or in meetings with government personnel. · All documents or reports produced in performance of the contract are suitably marked as contractor products or that contractor participation is appropriately disclosed. · Special Instructions/Requirements The Government shall have unlimited rights in all data or information (electronic or otherwise) designed, developed or used by contractor personnel in the performance of this contract. No data provided to, or developed by, the contractor shall be used for any purpose other than this contract without the prior approval of the NASWI Command Chaplain or CNRNW PAO. All information (data files, hard copy reports, and analysis) becomes the property of the government and the contractor shall return them to government at the completion of the task. 80 QUALITY ASSURANCE PRACTICES The government will ensure compliance with requirements of this effort through the following activities: · Review of periodic progress reports and associated budget and cost data. · Progress reviews as scheduled. · Interim review and comment of draft deliverables. · Annual AMS review · Periodic site visits to observe contracted services
