- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number N6883613T0387 is issued as a request for quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through the latest edition of the Federal Acquisition Circular. This solicitation is issued on a total small business set a-side basis, NAICS Code 541618. CLIN 0001 SURVEILLANCE AUDITS, BASE YEAR 2 EA CLIN 0002 SURVEILLANCE AUDITS, OPTION YEAR ONE 2 EA CLIN 0003 SURVEILLANCE AUDIT AND RECERTIFICATION, OPTION YEAR TWO CLIN 0003AA SURVEILLANCE AUDIT 1 EA CLIN 0003AB RECERTIFICATION 1 EA SURVEILLANCE AUDITS FOR FLCJ CODE 300's ISO 9001:2008 QUALITY MANAGEMENT SYSTEM: Provide five semi-annual surveillance audits during the three year cycle at times to be determined by FLC Jacksonville Code 300. Surveillance audits are periodic audits of Code 300's ISO 9001 management system. The purpose of surveillance audits is to ensure the management system is being maintained during the effective period of registration, ensuring the management system is working efficiently and facilitating the rate of continuous improvement. A formal audit report will be issued after the audit. The report is designed to facilitate business improvements by reporting on the management system's status relative to the requirements of the standard and includes information relative to positive aspects of the system, opportunities for improvement and system non-conformances. Dates: Nov 2013, May 2014, Nov 2014, May 2015, Nov 2015 RECERTIFICATION AUDIT FOR FLCJ CODE 300'S ISO 9001:2008 QUALITY MANAGEMENT SYSTEM: Provide re-certification audit at the end of the three year cycle. The goal of the re-certification audit is to assess the effectiveness of Code 300's management system to the requirements of the standard and Code 300's management system documentation. All audit activities must be planned prior to the audit. An audit plan is provided detailing the scope and objectives of the audit, including the audit team members, the processes to be audited and the timelines. The audit team will work to verify that the management system meets the requirements of the standard and Code 300's system documentation by assessing objective evidence. A formal audit report will be issued after the audit. The report is designed to facilitate business improvements by reporting on the management system's status relative to the requirements of the standard and includes information relative to positive aspects of the system, opportunities for improvement and system non-conformances. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 15 days before the contract expires. (End of clause) 52.217-9 OPTION TO EXTEND THE TERM OF THE CONTRACT (MAR 2000) (a) The Government may extend the term of this contract by written notice to the Contractor within 10 days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 30 daysbefore the contract expires. The preliminary notice does not commit the Government to an extension. (b) If the Government exercises this option, the extended contract shall be considered to include this option clause. (c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 36 months.
