Performance Work Statement Parts List RFQ Worksheet Period of Performance 1 November 2013 - 30 September 2014 CLIN 0001: Preventive Maintenance to be performed once monthly (11 MO) CLIN 0002: Reimbursable parts, labor and travel for repairs to fitness equipment IAW the Statement of Work, NTE $13,600.00 (13,600 DO), see attached parts listing Period of Performance 1 October 2014 - 30 September 2015 CLIN 1001: Preventive Maintenance to be performed once monthly, Option Year One (12 MO) CLIN 1002: Reimbursable parts, labor and travel for repairs to fitness equipment IAW the Statement of Work, Option Year One, NTE $13,600.00 (13,600 DO), see attached parts listing Period of Performance 1 October 2015 - 30 September 2016 CLIN 2001: Preventive Maintenance to be performed once monthly, Option Year Two (12 MO) CLIN 2002: Reimbursable parts, labor and travel for repairs to fitness equipment IAW the Statement of Work, Option Year Two, NTE $13,600.00 (13,600 DO), see attached parts listing Period of Performance 1 October 2016 - 30 September 2017 CLIN 3001: Preventive Maintenance to be performed once monthly, Option Year Three (12 MO) CLIN 3002: Reimbursable parts, labor and travel for repairs to fitness equipment IAW the Statement of Work, Option Year Three, NTE $13,600.00 (13,600 DO), see attached parts listing Period of Performance 1 October 2017 - 30 September 2018 CLIN 4001: Preventive Maintenance to be performed once monthly, Option Year Four (12 MO) CLIN 4002: Reimbursable parts, labor and travel for repairs to fitness equipment IAW the Statement of Work, Option Year Four, NTE $13,600.00 (13,600 DO), see attached parts listing Option for Execution of 6 Month Extension of Services IAW 52.217-8 CLIN 5001: Preventive Maintenance to be performed once monthly, (6 MO) CLIN 5002: Reimbursable parts, labor and travel for repairs to fitness equipment IAW the Statement of Work, NTE $7,600.00 (7,600 DO), see attached parts listing This synopsis and solicitation is prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quote (RFQ). Submit written offers (oral offers will not be accepted), on RFQ F1R37C3178A01. Quotes will be evaluated and an award will be made on the basis of the lowest evaluated price technically acceptable. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-69. All firms or individuals responding must be registered with the System for Award Management (SAM) IAW FAR 52.204-7. This procurement is being issued as a 100% Total Small Business Set-Aside. North American Industrial Classification Standard 811490, size standard, $7.0 million, applies to this procurement. Performance will be on DYESS AFB, TX 79607-1581. "Notice to Offerors: Funds are not available for this effort. No award will be made under this solicitation until funds are available. The Government reserves the right to cancel this solicitation, either before or after the closing date. In the event the Government cancels this solicitation, the Government has no obligation to reimburse an offeror for any costs." Prices must be good through 1 October 2013. The provisions that apply to this solicitation are as follows: • FAR 52.212-1, Instructions to Offerors-Commercial • 52.212-2 -- Evaluation -- Commercial Items. (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: both the parts list and the Request for quote worksheet will be evaluated on lowest price technically acceptable. (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. All questions pertaining to this requirement must be submitted and received no later than 1:00 p.m. (CST), 7 October 2013. Responses will be posted by 4:00 p.m. (CST), 9 October 2013.
