Description
RFQ Response Cover Sheet Basis For Award 52.212-2 Addendum Instructions to Offerors 52.212-1 Addendum Statement of Work THIS IS A COMBINED SYNOPSIS/SOLICATION for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only publication. Solicitation number M00681-13-T-0110 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-69 effective 3 September 2013. This procurement is 100% set-aside for small business. The applicable North American Classification System (NAICS) code for this procurement is 334510; size limitation of 500 employees. This requirement is being procured utilizing FAR Part 13 procedures. The Medical Logistics Company, 1 st Supply Battalion of Camp Pendleton, California requires service to calibrate and repair (18) Impact Model 754M Ventilators in accordance with the Statement of Work entitled Statement of Work Calibration and Repair of Biomedical Equipment.
