Description
1)This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This notice constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. 2)This solicitation, VA249-13-Q-0744, is issued as a request for quote (RFQ) Firm Fixed Price Contract. 3)This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-69. 4)The associated North American Industrial Classification (NAICS) code for this procurement is 811310 with a small business size standard of $7.0 Million. 5)This requirement is 100% set aside for small business. 6)This combined synopsis/solicitation is for the following commercial service to include parts, labor, and supervision: To provide rental of five (5) deionized tanks as well as maintenance on a Culligan Complete Multiple-Process Treatment System. 7)Period of Performance: Base Year with option to renew four (4) additional years. Total contract not to exceed five (5) years. In order to be considered responsive, the offeror must submit a quote on all itemized items and total base year including all option years pricing as provided in the attached document. 8)Quote Due Date: Offerors must submit all quotes to Katrina Cruz, via e-mail, katrina.cruz@va.gov, no later than 4:00PM EST on Monday September 30, 2013. No late offers will be accepted. Quote must be on company letterhead and shall include the completed pricing form (attached), along with the name, address, telephone number of the offeror, and Duns & Bradstreet Number. The Huntington VA Medical Center intends to award a Firm Fixed Price Contract based on the Best Value Methodology of Lowest Price/Technically Acceptable (LPTA). Resultant contract will be awarded to Offeror whose quote conforms to this Combined Synopsis/Solicitation, is deemed technically acceptable, and provides the best value. 9)Questions & Answers: All questions and inquiries about this solicitation must be in writing via e-mail, katrina.cruz@va.gov on or before Friday September 27, 2013, 4:00PM EST.
