Delivery locations / Quantities COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS General Information Document Type: Combined Solicitation/Synopsis Solicitation Number: HE125420130260348 Set Aside: None NAICS Code: 339112 Contracting Office Address Department of Defense Education Activity (DoDEA) 4800 Mark Center Drive Alexandria, VA 22350 Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, "Streamlined Procedures for Evaluation and Solicitation for Commercial Items," as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. This solicitation is a request for quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-69. The associated North American Industrial Classification System (NAICS) code for this procurement is 339112 with a small business size standard of 500. DoDEA (4800 Mark Center Drive Alexandria, VA 22350) is seeking to purchase: 1) Phillips HeartStart FRx AED School Value Packages or equivalent kits, which include one each of the following: • Phillips HeartStart FRx AED (P/N: 861304); • Responder Brand Premium Compact Size AED Wall Cabinet (P/N: AMP147-14RSM); • Phillips HeartStart Semi-Rigid Carry Case (P/N: 989803139251); • Phillips FRx Infant/Child Key (P/N: 989803139311); • Phillips Fast Response Kit Pouch (P/N: 68-PCHAT); • Phillips AED V-Style Wall Sign (P/N: 989803170921); • AED Check Tag (P/N: AMP3478); • AED Equipped Facility Window/Wall Decal (P/N: AMP1818) 2) Phillips Trainer Pads (for use with the HeartStart FRx AED) (P/N: 989803139271) or equivalent. 3) Phillips Training Toolkit (for use with the HeartStart FRx AED) (P/N: 989803139321) or equivalent. If quoting equivalent products, offerors shall provide supporting documentation with their quote that clearly demonstrates how the proposed item(s) meet(s) the salient characteristics shown below. In order be an equivalent product, the proposed item must contain all of the following salient characteristics of the specified item: • An AED battery with a minimum of 200 shock or at least 4 hours of operating time • Defibrillator pads with at least a two-year life from date of manufacture • A single defibrillator pad set that can be used for an adult, child, or infant • A defibrillator unit that can be easily switched for use on an adult or child • A defibrillator that is ready to use out of the box • A defibrillator that has a training mode with pre-programmed scenarios for practice use • Defibrillator pads (can be an additional training set) that do not conduct energy during the unit's training mode • Kit includes an operator's manual and quick reference cards • Kit includes at least a semi-rigid carrying case that is compatible with the proposed AED unit • Kit includes a quality check tag • Kit includes a metal wall cabinet with at least an alarm and a strobe light that is compatible with the proposed AED unit • Kit includes an AED projection wall sign and window/wall decal • Kit includes a first responders kit that includes at least: one (1) full-sized pocket style mask, two(2) pairs of non-latex gloves, one (1) pair of emergency shears, One (1) medical prep razor, one (1) absorbent towel • Kit includes an instructor and student training in video or electronic format that is consistent with AHA 2010 guidelines All interested companies shall provide a quotation for the following: Supplies Line Item Description Quantity 0001 AED School Value Kit 112 0002 Trainer Pad 50 0003 Training Toolkit 50 Quoted prices shall include all shipping and handling costs. See Attachment 1 for delivery quantities and locations for the items listed as Line Items 0001 through 0003 above. Delivery is required no later than 30 days after receipt of order. All shipping is FOB Destination. Quote Submission Submissions shall be received no later than 8:00am EST, 29 September 2013. Late submissions shall be treated in accordance with the solicitation provision at FAR 52.212-1(f). All quotations shall be sent via e-mail to carrie.moore@hq.dodea.edu. The Contracting Officer will respond to each offeror's quote submission with a receipt email. If a receipt email is not received by the offeror, then the Contracting Officer did not receive your quote and your offer may not be considered for award. Contract Award Award shall be made to the offeror whose quotation is the lowest-priced, technically acceptable offer. In order to be considered technically acceptable, an offeror's quotation shall include all of the items (either the brand name/part number specified above or an equivalent product meeting the salient characteristics provided above) and quantities described above.
