- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and written solicitation will not be issued. This solicitation is issued as Request for Quotation (RFQ) No. W15QKN-14-T-0005. The North American Industry Classification System Code (NAICS) for this acquisition is 721110 with a size standard of $30 m. This requirement is 100% set aside to small business. Prospective contractors shall be registered in System for Award Management, SAM, prior to or on date of receipt of quotes. A contractor can locate registration information at https://www.sam.gov/portal/public/SAM/ A DUNS (Dun and Bradstreet) number is required in order to register. All invoices subject to payment by the Defense Accounting and Finance Office shall be submitted through Wide Area Work Flow. You can register at https://wawf.eb.mil/ BASIS OF AWARD: Award shall be made to a single offeror lowest price technically acceptable. Quotes must include prices for each item listed in order that quotes may be properly evaluated. Failure to do this shall be cause for rejection of the entire quote. Award shall be made to that responsible, responsive offeror whose price is lowest, and technically acceptable. Technical Acceptability is 1)Facility Assessment consisting of cleanliness 2) Hotel must be FEMA approved, 3) all rooms must have access from inside the hotel and 4) Hotel location radius shall be within 42 miles of Wilkes-Barre Scranton Regional airport, by taxi, car service or airport shuttle. The distance will be calculated by utilizing commercial search engine; mapquest. Whichever website site that yields the least distance will be utilized. Price - Line Item pricing should include all applicable taxes, fees, gratuities, commissions, services charges, etc. The federal government is tax exempt. Room rates shall not exceed the government travel per diem rates in accordance with Joint Federal Travel Regulation (JFTR). Lodging per diem rate for Monroe County PA is $83per night/pp Your offer is due not later than Monday, 07 October 2013 at 3:00 PM Eastern Standard Time (EST). Attachment 1, Solicitation, shall be used for price quotes and submitted along with a cover letter on company letterhead signed by an individual authorized to sign as an agent of the offeror, with your company's CAGE code, DUNS and Federal ID number. Third party contractors must provide a Point of Contact (POC) that is employed by the contractor to be at the facility for the entire event. The name of the POC and telephone number must be submitted with their quote/offer. The Third Party contractor must submit an executed agreement with the hotel with their quote. The hotel selected must remain constant throughout the term of the contract.
