Description

COMBINED SYNOPSIS/SOLICITATION (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) This solicitation is issued as a Request for Proposal (RFP), referencing VA249-14-R-0001. Award will be made based on the best value to the Government. (iii) This is a solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-69. This solicitation document incorporates attached Sections B, C, D, and E. (iv) This is an unrestricted procurement with preference provided for small businesses as noted in the Evaluation Factors at E.10. FSC code is Q522. NAICS code is 621111, and Size Standard: $10 million. (v) See the attached Price/Cost Schedule at B.7. (vi) See the attached Statement of Work (SOW) at B.6. (vii) Period of Performance includes the Base Period of 16 October 2013 through 15 October 2014 and one (1) subsequent, 6-month Option Period for 16 October 2014 through 15 April 2015. Performance will take place at Memphis VA Medical Center, 1030 Jefferson Ave, Memphis TN 38104-2127. (viii) FAR 52.212-1, Contract Terms and Conditions-Commercial Items, applies to this acquisition; as well as the following addenda: N/A, no addenda to subject clause. (ix) See VAAR 852.273-73 Evaluation - Health-Care Resources (Jan 2003) at E.10. (x) Complete 52.212-3 Offeror Representations And Certifications--Commercial Items (Aug 2013) at E.2. (xi) FAR 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition; as well as the following addenda: N/A, no addenda to subject clause.
