- Wage Determination Medical Waste Schedule This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is 246-14-Q-0001 for Medical Waste Pickup and Disposal Services. This solicitation is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-70. This acquisition is full and open. The NAIC code is 562211 and the small business size standard is $35.5M. The FOB point is Claremore Indian Hospital, 101 South Moore Avenue, Claremore, Oklahoma 74017. Evaluation will be based on price and past performance. Offerors are to submit 3 references with current contact information: Name of Contact, Company Name, phone number and/or e-mail address. Purpose of the project: The purpose of this contract is for a contractor to pick up and dispose of regulated medical waste which includes sharps, chemo and other biological wastes for the Claremore Indian Hospital, 101 S. Moore, Claremore, OK 74017. 2. Background Information: Regulated medical wastes are generated by the Claremore Indian Hospital and must be disposed of in accordance with federal and state regulations. 3. Detailed description of the technical requirements: Contractor shall provide service to pick up and properly dispose of medical waste which includes sharps, chemo and other biological wastes. Contractor shall provide leak proof disposable waste containers with liners, in accordance with the Department of Transportation guidelines. The Contractor shall provide proper handling of waste, until incineration in a manner not to reflect back to the Claremore Indian Hospital. The Contractor shall provide training for the health care workers in determining what are regulated medical waste and the handling of such waste. Initially 14 medical waste containers will be needed with additional containers available when telephone contact is made by the Contractor Officer's designee. 4. Reference Material: The Contractor shall comply with all Federal Regulations including but not limited to Federal Environmental Protection Agency/Resources Conservation and Recovery Act (RCRA) and Department of Transportation regulations. Contractor shall comply with hospital's safety policies which can be obtain from the Safety Officer. 5. Level of Effort: The Contractor shall provide the Claremore Indian Hospital a letter indicating the employees working for the Contractor has had adequate training in handling regulated medical waste. 6. Special Requirements: The Contractor shall maintain insurance coverage to protect the U.S. Government against all claims arising from the handling of the waste from the Claremore Indian Hospital to the point of incineration. Contractor shall maintain general liability insurance in amounts ordinary and customary for their lines of business and level of activity. The USPHS Claremore Indian Hospital is a United States Federal government facility that is a tobacco/smoke free environment (building and grounds). No tobacco/smoking will be tolerated at this facility. Contractor shall not restrict or compromise fire safety emergency egress or patient flow entry/exit of the building. Any restrictions or compromise for safety must be coordinated with the Facility Manager. Access to building and parking shall be managed by the Facility Management Department. Distribute copies of any applicable documents as follows to: a. David Bales, Safety Officer, Claremore Indian Hospital b. Carl Hunter, Director of Support Services, Claremore Indian Hospital 7. Deliverables and reporting requirements: • Period of performance November 1, 2013 through October 31, 2014. • Pick up and dispose of regulated medical waste which includes sharps, chemo and other biological wastes a minimum of two (2) times per week. • Provide 14 Department of Transportation compliant leak proof disposable waste containers with liners with additional containers available when telephone contact is made by the Contractor Officer's designee. • Provide regulated medical waste compliance services including tracking /manifest method of labeling of each container for transport and disposal/incineration of waste with copies of manifest/evidence of proper disposal to the Claremore Indian Hospital to the following: a. David Bales, Safety Officer, Claremore Indian Hospital b. Carl Hunter, Director of Support Services, Claremore Indian Hospital CLAUSES INCORPORATED BY REFERENCE: FAR 52.204-9 Personal Identity Verification of Contractor Personnel (Jan 2011); FAR 52.223- 6, Drug Free Workplace (May 2001); FAR 52.232-18, Availability of Funds. (APR 1984) HHSAR 352.242-71 Tobacco-Free Facilities (January 2006) In accordance with Department of Health and Human Services (HHS) policy, the Contractor and its staff are prohibited from using tobacco products of any kind (e.g., cigarettes, cigars, pipes, and smokeless tobacco) while on any HHS property, including use in personal or company vehicles operated by Contractor employees while on an HHS property. This policy also applies to all subcontracts awarded under the contract or order. The term ‘‘HHS properties'' includes all properties owned, controlled and/or leased by HHS when totally occupied by HHS, including all indoor and outdoor areas of such properties. Where HHS only partially occupies such properties, it includes all HHS-occupied interior space. Where HHS leases space in a multi-occupant building or complex, the tobacco-free HHS policy will apply to the maximum area permitted by both law and current lease agreements.
