STATEMENT OF WORK (SOW) IFCAP Tracking Number:508-14-1-2043-0008 Follow-on to Contract and Task Order Number:Not applicable 1.Contract Title. C. difficile PCR test kits, supplied by Vendor. 2.Background. Vendor will supply C. difficile PCR reagent test kits and transport swabs for the testing of C. difficile in VAMC patient stool specimens. This is a FY2014 contract for Vendor C. difficile PCR reagent testing. 3.Scope. Vendor will supply C. difficile PCR reagent test kits and transport swabs for the testing of C. difficile in VAMC patient stool specimens. This is a FY2014 contract for Vendor C. difficile PCR reagent testing. Standing order to be delivered as noted in the delivery schedule. KITS and PACKAGES SHOULD NEVER BE ORDERED OR SHIPPED ALL AT ONCE. These kits, ordered by the Microbiology Laboratory of the Pathology & Laboratory Medicine Service line, are to be shipped according to the delivery distribution list.. C. difficile PCR reagents will be used to perform testing on VAMC patient specimens. This C. difficile PCR test is an extraction amplification and detection process fully automated with-in a cartridge, and integrated with automated real-time 6 color PCR system. This is an estimated cost for FY2014. Subject to availability of FY2014 funding. This is a standing order to be delivered per month as noted in the delivery schedule. 4.Specific Tasks. Vendor will supply C. difficile PCR reagent test kits and transport swabs for the testing of C. difficile in VAMC patient stool specimens. This is a FY2014 contract for Vendor C. difficile PCR reagent testing. Standing order to be delivered as noted in the delivery schedule. KITS and PACKAGES SHOULD NEVER BE ORDERED OR SHIPPED ALL AT ONCE. These kits, ordered by the Microbiology Laboratory of the Pathology & Laboratory Medicine Service line, are to be shipped according to the delivery distribution list. 5.Performance Monitoring Items will be checked in by Microbiology Laboratory staff. The items will be scrutinized for expiration date, as well as for proper quality control performance. Outliers will be reported to the Technical Services Department at Vendor for no charge replacement. 6.Place of Performance. These kits, ordered by the Microbiology Laboratory of the Pathology & Laboratory Medicine Service line, are to be shipped to the Microbiology Laboratory Rm 2B184 after receipt in the Atlanta VAMC warehouse. 7.Period of Performance. Start date: October 1, 2013 End date: September 30, 2014
