Description

This is a Sources Sought/Request for Information (RFI) notice for information only. This is not a solicitation for proposals and no contract will be awarded from this announcement. This RFI is issued solely for information and planning purposes and does not constitute a solicitation. Responses to this RFI will not be returned. It is the intent of the Contracting Office to use this for market research purposes. The Government shall not reimburse the costs of preparing responses to this request for information. The Department of Veterans Affairs Network Contracting Office is seeking potential small business sources. The Government anticipates issuing a commercial Request for Quote (RFQ) for a Dialysis Purification Water Treatment System in support of the Kansas City VA Medical Center. In preparation for future procurement for the VISN 15 Network, the Network 15 Contracting Office is performing market research to gain knowledge of potential sources and their size classification (service disabled veteran owned small business, veteran owned small business, hub zone, 8(a), small disadvantage business, woman-owned small business, small business, or large business) relative to NAICS 333318 that can offer the same or similar system listed in the attached Statement of Work. Please submit statements of capabilities electronically to paul.hemenway@va.gov no later than Monday October 17, 2013, 1530 CST. Phone inquiries will not be accepted. Please reference "KC VAMC Dialysis Purification Water Treatment System" in the subject line of your e-mail. Vendor Checklist for this announcement, please respond in the following format. 1. Vendor name: 2. Address: 3. Name, Email and phone number of primary contact: 4. DUNS Number: 5. Business size: 6. FSS Contract Numbers: provide contract numbers and indicate your ability to offer the requested items 100% through a FSS contract on the attached sample statement of requirements. Indicate which Schedules and SINs your contract is listed under and the FSS contract price for the items on contract. If your company is unable to offer this requirement via a FSS contract, then simply put N/A here. 7. A listing of experience in work similar in type and scope to include contract numbers, project titles, dollar amounts, and an explanation of the relevancy of the project. 8. Capabilities Statement: provide model numbers, standard features and options of equipment that can be offered. If there are any specifications that your product cannot meet, but are able to provide a comparable alternative, this is acceptable and encouraged as well. 9. Miscellaneous Comments: provide comments regarding the statement of requirements that may be pertinent to the improvement of the statement of requirements.
