- This is a Sources Sought Notice (Request for Information). The Government intends to establish a multiple Blanket Purchase Agreement (BPA) for various Cardiovascular Supplies for the Department of Surgery at Tripler Army Medical Center, Hawaii. The list includes, but is not limited to the attached product list. Calls will be placed against the BPA as supplies are required. Period of performance will be effective 15 NOV 2013 through 14 NOV 2018. The NAICS code is 339112. Interested parties shall reply to Pacific Regional Contracting Office via email to Jadie Tom (karen.j.tom.civ@mail.mil). TELEPHONE REQUESTS WILL NOT BE HONORED. Please indicate your interest and/or ability to provide the all items listed, as well as a current price list for the items. For consideration, please reply NO LATER THAN 28 OCT 2013 at 10:00 AM Hawaii Standard Time (HST) or 4:00 PM EST. Contractor MUST BE ABLE TO PROVIDE 100% of the item(s) listed.
