- This is sources sought announcement only. The purpose of this notice is to conduct market research and obtain information from qualified potential offerors. No formal solicitation document exists at this time. The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this sources sought synopsis request. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to this information if any solicitation is issued. Salem VAMC (SVAMC), Salem, Virginia has a recurring need for a contractor to furnish transitional housing for the beneficiaries of the Department of Veterans Affairs, Salem VA Medical Center (VAMC). Temporary and/or emergency residential housing will be located in the Salem, Virginia community or within a 25 mile radius of the Salem VAMC. The applicable NAICS Code is 623220 with a size standard of $14.0 Million. THIS IS NOT THE COMPLETE DESCRIPTION LIST: Contractor shall provide Residential Mental Health, Therapeutic and Rehabilitative Services to include Community Based Health Care Services for fifteen (15) beneficiaries of Homeless Vet Contract Beds for the Department of Veterans Affairs Medical Center (VAMC) Salem, Virginia 24153, which consists of: 3.0 REQUIREMENTS The contractor shall furnish each veteran authorized care under this contract with the following basic services: 3.1 Contractor will provide segregated and gender-specific bedroom and bathroom access, with sufficient locks on bedroom and bathroom doors. Contractor will provide adequate security measures to include sufficient monitoring and restriction of access by residents in segregated areas. 3.2 Room and Board to include a bed and other furnishings such as a dresser, storage, and linen for the bed. 3.3 Access to kitchen for preparation of two meals (breakfast and supper) a day, 7 days a week and availability of nutritious snacks between meals and bedtime for those requiring or desiring additional food, when it is not medically contraindicated. 3.4 Laundry facilities for residents to do their own laundry or to have laundry done. 3.5 The total number of beds required is (15). The maximum time allowed for any one (1) homeless Veteran to reside at this facility shall be (4 months), on a case by case bases not to exceed (6 months). 3.6 In rare circumstances, request for readmission to residential contract housing will be reviewed by COR and Social Work Chief of Services. 3.7 Therapeutic and Rehabilitative Services determined to be needed by the individual patient in a plan developed by the contractor with input from the patient and the VA HCHV case manager. Services, which the contractor must be able to furnish, include: 1.Structured group activities, including physical activities as appropriate. 2.Instruction in health and personal hygiene. 3.Monitoring of medications, including locked drawers for storage of medication. 4.Supportive social services, in collaboration with the HCHV case managers, or other contract resources. 5.Assistance in learning and development of responsible living patterns to achieve a more adaptive level of psychosocial functioning upgraded social skills and improved personal relationships. 6.Support for an alcohol/drug abuse free lifestyle by maintaining a drug and alcohol free environment. 7.Assist in arranging transportation to care at the VA Medical Center and other appointments. 3.8 Unless specifically excluded in this contract, the per diem rate established will include the services listed in the document and will also include all services normally provided other patients by the facility without extra charge. 3.9 Contractor will not be permitted to serve as payee representative or fiduciary of veteran's Social Security, Service-Connected and/or Non-Service Connected benefits, either as individual or organizational payee. Responses to this sources sought notice should include the following: name and address of company; business size (large or small); identify whether or not firm falls into any of the following socioeconomic categories - 8(a), HUBZone, Service Disabled Veteran-Owned; brief capability statement of qualifications and any other pertinent information, which demonstrates ability to meet the above requirement.
