Description
The VA Black Hills Health Care System will be seeking offers to submit price proposals to provide MRI coach transport services per the following statement of work. STATEMENT OF WORK 1.Contractor shall provide a tractor and driver to transport the Government furnished MRI coach between the VA Black Hills Health Care System (VABHHCS) Fort Meade (FM) campus and the Indian Health Service Eagle Butte Hospital (IHSEBH). The coach weighs approximately 63,000 pounds fully loaded and is 48 feet in length. 2.Contractor shall provide liability and collision insurance on the MRI coach during transport, along with contents, to include replacement value not to exceed $2,000,000 per coach. Loss of use (lease of a similar unit) shall be required after the 7th day of being out of service due to damage caused by driver negligence. Contractor driver(s) shall provide proof of this insurance, as well as full Department of Transportation (DOT) approved liability insurance coverage within ten (10) days after award. 3.Contractor shall provide all supplies associated with the movement, management, and maintenance of the coaches. During normal VA duty hours, Monday through Friday, 8:00am to 430pm, government holidays excluded, VA Facilities Management will be advised by the Contractor of any coach equipment failure or preventative maintenance due to be performed and the VABHHCS will schedule such maintenance. Contractor driver may be required to transport coaches to place of repair. If repair is required outside the normal VA duty hours and the estimate for the repairs and transport to repair site is less than $1,000.00 the Contractor will be responsible to complete the repair and ask for reimbursement. Contractor shall contact the Contracting Officer's Technical Representative (COTR) of intention of repair to receive authorization before repairs are completed. Contractor shall provide a copy of the invoice for reimbursement purpose. Reimbursement will be made under a separate purchase order away from this contact. If the repair shop agrees to invoice the VA, then the VA will reimburse the repair shop directly. 4.Contractor shall be responsible to maintain the tractor in accordance with DOT/State and Federal regulations. The VA will be responsible for the expense of items to maintain the coach in accordance with DOT/State and Federal regulations. 5.Contractor shall provide transportation services in accordance with the following, ESTIMATED typical schedule: a.One (1) or two (2) instances of transport per month. b.Contractor will be provided with transportation schedule at least 30 days in advance. c.Changes to schedule may be made within the 30 day window with mutual agreement by the Contractor and the VABHHCS. d.Contractor will invoice the VABHHCS monthly for instances of transport of the coach to the IHSEBH site. 6.Contractor shall set up the coach at the site by leveling the coach, connecting the coach to provide electricity, data ports, and telephone lines. Contractor shall complete all set up procedures and tests for functionality by 8:00am at the IHS Eagle Butte Hospital site. The VABHHCS and IHSEBH will provide connections and system testing protocols and contact personnel for test failures. Fuel for the generator will be available at VABHHCS site. Contractor shall park coach at VABHHCS sites only when not being transported between sites for medical use. Coaches shall not be parked at Contractor's site at any time. 7.Contractor shall not be responsible to perform when roads are closed by competent authority or when the Contractor determines the transport is too risky. Contractor shall notify the COTR immediately in this instance. The VABHHCS will not be liable for additional costs incurred by Contractor as a result of road closures. Depending on the weather conditions the coach transport between sites may be delayed and additional patient scheduling may occur at the site the affected that day, and/or for road closure days lost, alternative days may be scheduled to make up the loss, with mutual agreement between Contractor and the VABHHCS. 8.Contractor shall comply with all safety requirements such as posting hazardous signs around the coach and shall provide proper protection for the electrical power cable (under a metal cover if it crossed a sidewalk), etc., to assure patient, employee, Contractor, traffic, and pedestrian safety. 9.Contractor driver shall be appropriately licensed to operate the tractor in accordance with all federal, state, and local statues. Verification and validation of licensure is required. 10.Contractor shall maintain current license, insurance, inspections, adjustments, calibrations and certifications at all times. Contractor shall provide the VABHHCS with copies of these documents, within ten (10) calendar days after award and within ten (10) calendar days of inspection/adjustments/calibrations/change of personnel whenever such occurs. 11.For any loss in MRI scanning time at any of the coach destination sites due to Contractor inability to perform, the time lost will be deducted from the Contractor monthly reimbursement based on a calculated hourly rate for the Contractor's services (monthly/instance rate divided by the number of hours lost). After the second failure to perform during the current contract period, the Contractor will be assessed $1,000.00 per occurrence. OTHER CONTRACTOR SERVICES AND REQUIREMENTS 1.Contractor shall provide tractor(s) identified solely for this MRI coach transport purpose. 2.Contractor shall transport coach to and from repair location upon request at the VABHHCS expense. 3.Contractor shall be responsible for all costs of operating the tractor(s), including fuel and maintenance. 4.Contractor shall obtain fuel for the generator from VABHHCS Facilities Management at the individual VA sites and ensure that the generator tank is always full. 5.Contractor shall ensure tractor exhaust is side configured so as not to enter air exchange in coach during transport. 6.Contractor's staff names and proof of license and insurance shall be provided and each must have and provide documentation of annual TB screening, report and new conversations and receive appropriate immunizations as required by the Government. 7.One day per quarter, the coach will be down for preventative maintenance at no penalty to the Contractor. 8.The following Federal holidays are observed by the Department of Veterans Affairs and typically MRI services will not be offered on these days, however, MRI may be scheduled with mutual agreement between the Contractor and the VABHHCS. a.New Year's Day1st day of January b.Martin Luther King's Birthday 3rd Monday in January* c.President's Day3rd Monday in February* d.Memorial DayLast Monday in May e.Independence Day4th of July f.Labor Day1st Monday in September g.Columbus Day2nd Monday in October* h.Veteran's Day11th of November* i.Thanksgiving Day4th Thursday in November j.Christmas Day25th of December NOTE: Any of the above holidays falling on a Saturday will be observed on the preceding Friday, holidays falling on a Sunday will be observed on the following Monday. The four (4) Federal holidays marked with an asterisk (*) are days when the Contractor may be required to take coach to repair shop or VA shop for regular preventative maintenance or unexpected repair. The solicitation will be posted on the Federal Business Opportunities (FBO) website on approximately October 23, 2013. This will be small business set aside procurement.
