U.S. DEPARTMENT OF LABOR, WAGE DETERMINATION NO. 2005-2043, REV. 16, DATED 19 JUN 2013 APPLICABLE CLAUSES IN FULL-TEXT DFARS 252.209-7998 - REPRESENTATION REGARDING CONVICTION OF A FELONY CRIMINAL VIOLATION UNDER ANY FEDERAL OR STATE LAW (DEVIATION 2012-O00007) FA9301-14-R-0004 PRICING WORKSHEET MISSION-ESSENTIAL CONTRACTOR SERVICES PLAN PERFORMANCE WORK STATEMENT, DATED 23 OCT 2013 Solicitation number is FA9301-14-R-0004. The contemplated contract number is FA9301-14-D-0007. This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are requested and a written solicitation will not be issued. This is a request for proposals (RFP). This solicitation also uses the simplified acquisition procedures contained in Part 13. All responsible sources may submit a firm-fixed price (FFP) proposal which shall be considered by the agency. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-70, effective 30 Sep 2013 and DFARs Change Notice (DCN) current to DPN 20130909 effective 09 Sep 2013, and AFFARS Parts current through AFAC 2013 Baseline, effective 27 Aug 2013. The Government intends to award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (i) technical capability of the services offered to meet the Government requirement and (ii) price. NAICS is 812332, Flame Resistant Clothing Supply Services, with a small business size standard of $35,500,000. Delivery: Acceptance is FOB Destination, Edwards AFB, CA 93524. Funds are not presently available for this effort. No award will be made under this combined Synopsis/Solicitation until funds are available. The Government reserves the right to cancel this solicitation, whether before or after the closing date. In the event the Government cancels this solicitation, the Government has no obligation to reimburse an offeror for any costs. AFTC/PZIOC, Edwards AFB, CA is seeking to purchase: Please see attachment "Performance Work Statement," dated 23 OCT 2013, for the full statement of this requirement. REQUIREMENT SUMMARY: The contractor shall provide all management, labor; Flame Resistant (FR) rated shirts (long-sleeve), pants and coveralls, equipment and transportation necessary to perform the following service for the 412th TW/Civil Engineering Operations Section (CEOS) at Edwards AFB, CA. The contractor shall perform to the standards as described in the PWS, dated 23 OCT 2013, to conduct laundry service of contractor owned FR rated clothing. The contractor will receive, load, count, process, deliver and unload clothing and return all items tendered from the cleaning in accordance with 1.2.2. of the PWS, dated 23 OCT 2013. The service to be performed shall be "non-personal services." The FR rated clothing is a requirement to perform electrical maintenance per ETL 06-9 (Arc Flash), AFI 32-1064 (Electrical Safe Practices) and NFPA 70E (Electrical Safety in the Workplace). Personal protective equipment (PPE) that provides appropriate arc flash protection is required for all personnel working on or near exposed energized electrical equipment. EVALUATION OF PROPOSALS AND BASIS OF AWARD: Offerors are instructed to include enough information in their proposals to facilitate a thorough technical evaluation by the Government. Technical evaluation will be made based on a complete response in which the vendor shall provide the appropriate materials as described in the Performance Work Statement (PWS) including an appropriate service schedule for the supply, receipt, loading, counting processing, delivery/unloading of the required equipment. "Appropriate materials" shall be defined as those PPE items/quantities listed in the Performance Work Statement (PWS) dated 23 OCT 2013 and required per ETL 06-9, AFI 32-1064 and NFPA 70E. "Appropriate service schedule" shall be defined as a weekly schedule as described in the PWS. No partial quotes for the services listed above and detailed in the Performance Work Statement, dated 23 OCT 2013 will be accepted. FOB: Destination, Edwards AFB, CA. Award will be based on lowest price, technically acceptable (LPTA). Evaluation Factor 1: Price (note the 6 month option will be evaluated with the total award price but will not be exercised unless needed). Evaluation Factor 2: Technically Acceptable. "Technically Acceptable" will be rated on an "Acceptable / Unacceptable" basis. "Acceptable" will be defined as "Proposal clearly meets the minimum requirements of the solicitation." "Unacceptable" will be defined as "Proposal does NOT meet the minimum requirements of the solicitation." ***** PLEASE READ THIS SECTION THOROUGHLY ***** The following items MUST BE INCLUDED with your proposal to be considered Technically Acceptable: 1. Product Detail Sheet: Offeror must provide a listing of the PPE they propose on supplying for the duration of the contract. Details should include arc-flash and FR ratings. Photos of the PPE are not required, but may be submitted in addition to the Product Detail Sheet. Offerors that fail to submit this info or PPE that does not meet the standards set forth in the PWS shall be rated "Unacceptable". 2. Service Schedule: Offeror must provide a service schedule in accordance with paragraph 1.2 and 4.1-4.3 of the PWS. The service schedule may be a simple statement that the offeror acknowledges and agrees to the schedule as defined in the PWS. Offeror's that fail to submit a service schedule or statement as described above in accordance with the PWS shall be rated "Unacceptable". 3. Mission Essential Contractor Services Plan: Offerors must complete the Mission Essential Contractor Services Plan. This plan shall be incorporated as an Attachment to the basic contract once awarded. The following areas of the plan require a response: 3.1. Paragraph 1 requires a fill-in response. 3.2. Paragraph 3(a-f) require full and complete responses. 3.3. Paragraph 6 requires a fill-in response 3.4. Complete the signature page section (all blanks shall be completed with the appropriate info.) Offerors that fail to submit or fully complete the Mission Essential Contractor Services Plan shall be rated "Unacceptable." 4. Pricing Worksheet: Offeror must fully complete the pricing worksheet. Offeror's that fail to submit or fully complete the pricing worksheet shall be rated "Unacceptable". An "Unacceptable" rating in any of the above areas will result in an "Unacceptable" rating for the Offeror's total proposal. Offerors must receive an acceptable rating in each area to be considered "Technically Acceptable." Pursuant to FAR 52.217-8, proposal pricing will be evaluated for the six month option period (1 Oct 2018 - 31 Mar 2019), and these prices will be the same as the per-unit CLIN pricing included in any award resulting from this solicitation.
