- Q - Orthopedic-Spine Surgeon The Department of Veterans Affairs, Network-9 Contracting Office (NCO-9), and the Robley Rex VA Medical Center, 800 Zorn Avenue, Louisville, Kentucky 40206-1433, intends to solicit, negotiate, and award a single firm-fix-price (FFP) contract for scarce medical services - 1.0-Full-Time Equivalent Employee (FTEE) Orthopedic-Spine Surgeons. Clinical services shall be provided at the contractor's facility and surgical services will be performed at the Robley Rex VA Medical Center. The contractor's facility where clinical services are to be provided under this solicitation must be within a 15-mile radius of the Robley Rex VA Medical Center, 800 Zorn Avenue, Louisville, Kentucky 40206-1433. The Authority for this procurement extends from Title 38 U.S.C. 1703 contracts for hospital and medical services. The solicitation will be issued on or about Friday, November 8, 2013, (this is subject to change), the solicitation package will contain the closing date and time for this solicitation. All information for this solicitation will be posted on www.fbo.gov/. Offeror's are advised that they are responsible for obtaining all documents and acknowledging any amendments, and should re-visit the Federal Business Opportunity (FBO) Website periodically to check for any changes to this notice, updates, or amendments without further notice from the issuing Network Contract Office. No other site shall be used to obtain these documents. In addition, potential offeror's are responsible for monitoring this site for the release of the solicitation package, any other pertinent information and for downloading a copy of the solicitation package. Performance period is expected to be one (1) year with two (2) one (1) year option periods with a base year estimate start date of December 1, 2013. The applicable North American Industry Classification System (NAICS) code for this procurement is 621111 and the size standard is $10.0-million. All interested firms as prescribed in Federal Acquisition Regulation (FAR) Provision 52.204-7 System for Award Management must be registered in the System for Award Management (SAM) database (https://www.sam.gov) and must have a current record in the online Representations and Certifications Application (ORCA) at www.bpn.gov. Written questions shall be submitted to John D. North III, Contracting Specialist, at John.North@va.gov after issuance of the solicitation. As prescribed in Federal Acquisition Regulation (FAR) Sub-part 12.204(b) the contracting officer may allow fewer than 15-days before issuance of the solicitation. In addition, FAR Sub-part 12.205(c) the contracting officer may allow fewer that 30-days response time for receipt of offers for commercial items. This notice is for informational purposes only and is not a request for submission of offers. The Government is not obligated to issue a solicitation as a result of this notice. No other information is available until issuance of the solicitation. No phone calls or e-mails to VISN-9 employees during the synopsis and solicitation period concerning this procurement - ALL questions shall be addressed through the assigned contract specialist. This is not a new requirement, the incumbent contractor currently providing this requirement in accordance with Performance Work Statement (PWS) has been acquired by another firm and will not exist after November 30, 2013. Reminder all small business that are a Service-Disabled, Veteran-Owned Small Business (SDVOSB) or Veteran-Owned Small Business (VOSB) must be registered and listed as "verified" in VIP (http://www.vetbiz.gov) in accordance with Title 38 U.S.C. §8127(d). In the event of an inconsistency between the synopsis and the Request for Proposal (RFP) the RFP will take precedence.
