- STATEMENT OF WORK A. General 1.Provide opiate substitution services for VA patients referred by the Providence VA Medical Center/OTP physician to include: a.All opiate substitution medications and pharmaceutical counseling. b.Copy of patient progress notes to document patient management and receipt of medication. 2.The contractor's place of treatment to which VA patients must report to receive contract services shall be available to provide contract services to VA patients from 7:00 am to 9:30 am, Saturdays, Sundays, federal holidays, and on dates of forecasted inclement weather that poses travel/safety concerns, per clinical judgment. B.Estimated Work Load 1.Upon contract award, the average number of patients to receive pharmaceutical care for regular opiate replacement treatment is projected to be approximately thirty (30) being dosed once or twice each week. This estimate may increase or decrease as determined by the VA. C.Patient Management Documentation and Communication with VA Clinical Staff: 1.Patients shall be informed by VA staff of this contractual arrangement for services. Only OTP clinical staff will schedule/refer VA patients for services with the designated contractor. The VA/OTP will contact the contractor telephonically and/or by fax, of all patients to be referred to the weekend medication treatment program at the time of the decision to refer the patient for services. The VA/OTP will provide documentation of referral to treatment which will include; a physical description of the patient, the identifying data as listed in section #7 of this document and will obtain and provide the contractor a completed consent form to release VA medical records protected by 38 U.S.C. 7332. At the time of the patient's initial visit to the contractor, the patient will present a copy of the referral documentation and show either the VA patient ID card or other agreed upon identification. The contractor shall also obtain and provide to the VA/OTP, a patient consent form to release information to the VA regarding care provided by the contractor. 2.A copy of contractor's patient management and treatment documentation shall be provided as requested. 3.Within the first regular business day following a weekend, the contractor shall contact by 9:00 am, a designated VA/OTP clinical staff telephonically at (401) 273-7100, ext. 2360/3978 or by fax at (401) 457-3324 to report any patient incident, adverse or untoward event, related directly or indirectly to patient treatment provided by the contractor. A copy of written documentation regarding the event shall follow the telephonic communication. 4.The contractor shall also communicate either by telephone or fax, the list of patients that both received their scheduled treatment or did not show for scheduled treatment. Again, this communication shall occur by 9:00 am of the first business day following the weekend. 5.The contractor shall notify the VA/OTP if a patient elects to refuse the opiate substitution medication. 6.For all other emergent needs or problems, the contractor my contact the OTP Clinical Director by paging (401) 325-8731 or telephoning the Providence VAMC Emergency Room at (401) 273-7100, ext. 2615 and request the psychiatrist on call. D.The VA/OTP Shall Provide the Following Information to the Contractor Upon Referral of New Patients: 1.Full patient name, social security number and date of birth. 2.A completed consent form to release VA medical records protected by 38 U.S.C. 7732. 3.Date of referral. 4.Appropriate clinical history (both medical and psychiatric). 5.Physical descriptions, address, phone number, and identifiable tattoos or scars of referred patient's. 6.Name and telephone number of VA/OTP contact. E.Accreditation, Regulatory and Staff Qualifications: 1.The contractor shall be accredited by, or have applied for accreditation by, either the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) or the Commission of Accreditation of Rehabilitation Facilities (CARF). A copy of this accreditation must be submitted with the solicitation response. 2.The contractor shall provide copies of safety programs and policies on how personnel monitor/measure patient quality/safety. The contractor shall provide copies of all training made available to staff. 3.The contractor shall provide a bi-annual report of performance improvement measures and quality indicators in use by the contractor that demonstrate the quality of patient care and patient safety. 4.The contractor shall provide a quarterly report on monitors during the quarter. 5.The contractor shall provide a report detailing the names and credentials of all staff. 6.The contractor shall comply with HIPAA regulations and execute a Business Associate Agreement (BAA) with the Dept. of Veterans Affairs. 7.The contractor must perform the required work in accordance with JCAHO and/or CARF standards. The contractor is required to develop and maintain the following documents for each contractor employee working within this contract: a.Credentials and qualifications for the job b.A current competence assessment checklist (an assessment of knowledge, skills, abilities and behaviors required to perform a job correctly and skillfully, includes knowledge and skills required to provide care for certain populations as appropriate). c.A current performance evaluation supporting the ability of the contractor employee to successfully perform the work required in this solicitation and listing of relevant continuing education for the past two years. d.The contractor shall provide current copies of these records at the time of contract award and annually on the anniversary date of contract award to the VA Contracting Officer's Technical Representative (COTR), or upon request, for each contractor employee working on the contract. e.The contractor shall have all licenses, permits, accreditation and certificates required by state and federal law. f.Licensed, registered and certified contractor employees shall follow the requirements of their licensure, registration, certification and their employing agency in the performance of their duties. Licensure shall be current and copies shall be submitted to the VA/OTP on request.
