Attachement 2 Provisions and Clauses Attachment 1 Performance Work Statement dated 4 November 2013 Attachment 3 DFARS 252.209-7994 PDF version of Solicitation FA8601-14-T-0002 SOLICITATION This is a solicitation for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Government reserves the right to award without discussions or make no award at all depending upon the quality and price reasonableness of offers received. An award, if any, will be made to the responsible offeror whose proposal conforms to the requirements of the solicitation and represents the best value to the Government, price and other factors considered. Requirement: The U.S. Air Force (AFLCMC/PZIOA) requires an experienced vendor to provide off-site, after-hours answering service for the Medical Treatment Facility (MTF) at Wright-Patterson AFB, Ohio. The anticipated award is Firm Fixed Price – Best Value. Additional detail on the requirements can be found in the Performance Work Statement (Attachment 1). Solicitation Number: The solicitation number for this Request for Proposal (RFP) is FA8601-14-T-0002. Set-Aside: This acquisition is a Small Business Set-Aside. The NAICS code for this acquisition is 561421 and the associated small business size standard is $7 million. This acquisition is going to be awarded full and open competition. Provisions and Clauses: This RFP incorporates FAR provisions and clauses in effect through Federal Acquisition Circular (FAC) 2005-70, effective 30 Sep 2013; Defense Federal Acquisition Regulation Supplement (DFARS) Publication Notice (DPN) 20130930, effective 30 Sep 2013; and Air Force Federal Acquisition Regulation Supplement (AFFARS) Air Force Acquisition Circular (AFAC) 2013 Baseline, effective 27 Aug 2013, and CPM 13-C-03. This solicitation is conducted as a commercial item procurement using FAR Part 12.6. Contract Line Items (CLIN): This RFP has four (4) CLINs. Detailed descriptions of these CLINs are located in the Provisions and Clauses (Attachment 2). Instructions for Submission of Quotes/Offers: Proposals shall be accompanied by a completed FAR 52.212-3 – Offeror Representations and Certifications, and adhere to FAR 52.212-1 – Instructions to Offerors – Commercial Items. All are incorporated in full text located in the attached solicitation document. Additionally, offerors are required to submit a completed DFARS 252.209-7994, which can be found at Attachment 3. IMPORTANT NOTICE TO CONTRACTORS: All prospective awardees are required to register at the System for Award Management (SAM) and to maintain active registration during the life of the contract. SAM can be accessed at https://www.sam.gov. Any award resulting from this solicitation will include DFARS Clause 252.232-7003, Electronic Submission of Payment Requests. Section 1008 of the National Defense Authorization Act of Fiscal Year 2001 requires any claims for payment (invoices) under DoD contract to be submitted in electronic form. Wide Area Workflow- Receipt and Acceptance (WAWF-RA) is the DoD system of choice for implementing this statutory requirement. Use of the basic system is at no cost to the contractor. Contractors must complete vendor training, which is also available at no cost at http://www.wawftraining.com. Prior to submitting invoices in the production system, contractors must register for an account at http://wawf.eb.mil/. Contract financing is NOT provided for this acquisition. The Government intends to award one firm fixed price (FFP) contract resulting from this solicitation to the responsible offeror whose proposal conforms to the solicitation and will be most advantageous to the Government. The Government intends to award without discussions and the Government reserves the right not to make an award at all. Consent to RFP Terms and Conditions: Submission of proposals in response to this RFP constitutes agreement by the offeror to all terms and conditions contained in this announcement and the attached solicitation document, which will be incorporated into the resulting contract. Period of Performance: Base Year 1 December 13 – 30 November 14 Option Year I 1 December 14 – 30 November 15 Option Year II 1 December 15 – 30 November 16 Six- Month Extension 1 December 16 – 31 May 17 Delivery Destination: Wright-Patterson Medical Center 881 Sugar Maple Drive Building 830 Wright-Patterson AFB, OH 45433-5529 Inspection and Acceptance: Both inspection and acceptance will be by the Government at Destination. Due Date and Point of Contact: Proposals must be received not later than 3:00pm, Local (Wright-Patterson AFB) Time on 12 NOVEMBER 2013.
