Description
This is a sources sought notice to determine the availability of potential small business sources capable of providing project laser support services to the Naval Medical Center, Portsmouth VA (hereafter referred to as the MTF ). The MTF requires Laser Support Services in their Main Operating Room (MOR). The service requirements encompass the provision of medical laser systems and medical laser systems specialists to operate the laser systems. Service will be ordered on an as needed basis; the Government will provide a list of scheduled procedures to the contractor 15 days prior to the requirement for service. The contractor shall recognize that the schedule provided may change within those 15 days. The Government estimates that about 1 time per year, 24-hours will be the maximum notice given to the contractor. The contractor must have the ability to provide full on-site support with a minimum 24 hour notice. The government reserves the right to request services in alternate treatment rooms located within the MTF proper. The laser systems specialists shall provide support of the following medical procedures. Colposcopy Condyloma ablation Dermal ablation Laparoscopic laser ablation Lithotripsy Stapedectomy Tympanomastoidectomy Vein laser therapy Vulva ablation The following Contractor tasks are included in the support of this requirement: Set-up and operate medical lasers in an operating room environment and provide full support for the requested laser services. Pre-work scrub, equipment set-up and calibration and inspection of the contractor s equipment by the MTF Biomedical Engineering Office Perform a general pre-procedure inspection of government owned laser systems and shall bring any discrepancies to the attention of the government Transport lasers and laser accessories to the MTF MOR Provide information to physicians and surgical staff on general laser safety and the proper operation of various medical lasers Maintain laser equipment properly and follow all safety regulations and procedures. Provide a general report of completed services and results to the MOR Provide laser operators who are certified and/or qualified to operate the following lasers: oLumenis Versapulse Powersuite Dual 80/100W. Laser for urology procedures, 2100 nm and 1060 nm wavelength, power output up to at least 100W. oLumenis Ultrapulse Encore. CO2 laser for dermatologic procedures, 10,600 nm wavelength, power output up to 60W-100W. oUltrapulse Encore Surgical. CO2 laser for surgical/dermatologic procedures, 10,600 nm wavelength, power output up to 60W-100W. oGreenLight HPS Laser System. KTP laser for urology procedures, 532 nm wavelength, power output up to 120W. oLaser for urology procedures, 980nm wavelength, power output up to at least 180W and both pulsed and continuous treatment modes. oLaser for urology procedures, 532 nm wavelength, power output up to at least 120W. oLaser for urology procedures, 532 nm wavelength, frequency-doubled, power output up to at least 180W. oCO2 laser for Ear, Nose, and Throat procedures (ENT), 10600nm wavelength, continuous and pulsed treatment modes, power output up to at least 20W during continuous mode. oHolmium:yttrium-aluminum-garnet (Ho:YAG) laser for urology, gynecology, gastroenterology, and podiatry, 2100nm wavelength, power output up to at least 60W. oCO2 laser for dermatological procedures, 10600nm wavelength, power output up to at least 60 W. oCO2 laser for transmyocardial revascularization procedures. oPotassium-titanyl-phosphate (KTP) laser, 532 nm wavelength, power up to 40W. oKTP laser for ENT procedures, 532nm wavelength, power up to at least 15 W continuous and up to at least 150W during pulsed mode operation. oNeodymium(Nd):YAG laser for urology, gynecology, gastroenterology, and podiatry, 1064 nm wavelength. oArgon beam coagulator for coagulation and hemostasis of bleeding tissues. Include disposable grounding pads, laparoscopic probes, and hand pieces. oFrequency-doubled, Q-switch Nd:YAG laser for tattoo removal, 532nm and 1064nm wavelength. Maintain and provide in good working order the following equipment and technology required to provide laser support services at the MTF: oLaser for urology procedures, 980nm wavelength, power output up to at least 180W and both pulsed and continuous treatment modes. oLaser for urology procedures, 532 nm wavelength, power output up to at least 120W. oLaser for urology procedures, 532 nm wavelength, frequency-doubled, power output up to at least 180W. oCO2 laser for Ear, Nose, and Throat procedures (ENT), 10600nm wavelength, continuous and pulsed treatment modes, power output up to at least 20W during continuous mode. oHolmium:yttrium-aluminum-garnet (Ho:YAG) laser for urology, gynecology, gastroenterology, and podiatry, 2100nm wavelength, power output up to at least 60W. oCO2 laser for dermatological procedures, 10600nm wavelength, power output up to at least 60 W. oCO2 laser for transmyocardial revascularization procedures. oPotassium-titanyl-phosphate (KTP) laser, 532 nm wavelength, power up to 40W. oKTP laser for ENT procedures, 532nm wavelength, power up to at least 15 W continuous and up to at least 150W during pulsed mode operation. oNeodymium(Nd):YAG laser for urology, gynecology, gastroenterology, and podiatry, 1064 nm wavelength. oArgon beam coagulator for coagulation and hemostasis of bleeding tissues. A firm fixed price contract is anticipated that will consist of a one year base period and two, one year option periods. The North American Industrial Classification System (NAICS) code for this acquisition is 541990 and the standard is $14 million. This is a non-personal service contract, so the contractor is required to have their own insurance, including Medical Liability Insurance, and Workers Compensation and Employer s Liability insurance. Quoters possessing the requisite skills, resources and capabilities necessary to perform the stated requirement are invited to respond to this sources sought notice via the submission of an executive summary, no more than three (3) pages in length. Responses must use twelve point (12pt) Times New Roman font. The submission should include the following information: 1) company name, address, and point of contact with corresponding phone number and e-mail address 2) DUNS number, business size and classification 3) relevant corporate experience information. Relevant corporate experience information is defined as experience (within the past five years) that is of the same or similar services to that which is described herein and a technical description of ability to meet the requirement (2 references). The corporate experience should include the applicable contract number, total number of FTE, contract period of performance, a brief description of the services provided AND the relevancy of the services to the proposed NMC requirement. A customer point of contact with corresponding telephone number and e-mail address should also be included. Standard brochures and/or paraphrasing of the Source Sought Notice will not be considered sufficient to demonstrate the capabilities of an interested party.
