Lutz VA Medical Center, Saginaw, Michigan. 506-14-1-2421-0058. 2.Nature and/or Description of the Action Being Processed: Opt-E-Man 100 mbs data services for the Aleda E. Lutz Health Annex, T3 Data Services for the Clare and Bad Axe CBOCS and T1 Data Services for the Saginaw Vet center. Period of Service 10/1/2013 to 3/31/2014. 3.Description of Supplies/Services Required to Meet the Agency's Needs: Continuation of Opt-E-Man data services for the Aleda E. Lutz Health Annex, T3 Data Services for the Clare and Bad Axe CBOCS and T1 Services for the Saginaw Vet center. This vendor has offered the best service, price and continuation of this existing service for next six months until the VA's Technology Acquisition Center consolidation of this service is in the government's best interest. 6.Description of Efforts Made to ensure that offers are solicited from as many potential sources as deemed practicable: FSS and the internet were searched. An intent to sole source was published to FBO with no responses. 7.Determination by the Contracting Officer that the Anticipated Cost to the Government will be Fair and Reasonable: $79,266 contract total. This price is fair and reasonable based on historical pricing for these services. PO 506S37079 for FY 13 services shows similar pricing for each line item. Services for FY 14: Opt-e-man data circuit $4,255/month, FY 13-$6,448/month. T3 Circuit Clare CBOC $3,740/month, FY13-$3,882/month. T3 Circuit for Bad Axe CBOC $4091/month, FY 13-$4,299/month.
