- Questions and Answers: 1. Is this a new or follow on requirement? If it is a follow on, please let us know the current contract number and contract value? Answer: This is a follow on requirment. Institute for Healthcare Improvement is the incumbent contractor and the total value of the Contract# HHSI236201000004C is $1,728,143.12. 2. Who is the incumbent? Is it still eligible to bid as a prime for this contract? Answer: The incumbent contractor will not bid on the follow on requirement. 3. I do see mention of ongoing work for the Quality & Innovation Learning Network in the areas of: 1) Care coordination/Care management 2) Advances access 3) Mobilize tribal and community based health programs 4) Behavioral health integration within the primary care setting 5) Cardiovascular Disease 6) Cancer Screening 7) Diabetes mellitus Comprehensive Care Would you be able to let us know if these are the topics for the IPC 5, and if they are not, would be able to provide the IPC 5 topics so we can accurately reflect our depth in the subject matter. Answer: The overall training that is provided is how to run a collaborative and be a part of a learning network using various models such as the Care Model, the Model for Improvement and Science of Improvement. The training is provided to health care teams and their leadership and most is done virtually. The content for the training for IPC 5 includes the broad training topics described in paragraph one as well as care teams and their roles and responsibilities, clinical information systems and data collection and management, executing and accelerating change, the Model for Improvement, overview of health care measures, aim development, empanelment and identification of the microsystem, optimization of the care team, roles of the teams and optimization of the teams, self-management support, tribe and community involvement and partnership, assessing best practices and development of patient centered medical homes. This is a highly complex network of teams located throughout the Indian Health system.
