Description
Statement of Work NIAIDNOI13A1653927 This is a combined synopsis/solicitation Notice of Intent for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number for this acquisition is NIAIDNOI13A1653927, and the solicitation is being issued as Notice of Intent. The incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC 05-70). This acquisition will be processed under FAR Part 13, Simplified Acquisition Procedures and is not a Small Business set aside. All responsible sources may submit offers that will be considered by this agency. The North American Industry Classification System (NAICS) Code for this acquisition is 811219, which has a size standard of $19.0 in millions of dollars. The National Institutes of Health, National Institute of Allergy and Infectious Diseases (NIAID), intends to procure on a sole source bases from Philips Healthcare (PENAC), for a one year preventive maintenance service agreement, for the M/A Silver Service Pk, iU22 Ultrasound Statement of work is included as an attachment. Philips Healthcare PENAC is the only known vendor or manufacturer that is authorized and able to provide certified expertise and technicians. Parts and accessories must be original so not to affect the manufacturer warranty. Contractor Requirements: The maintenance service is to include PENAC iU22 Ultrasound, site 47524707, serial# 034K2F. Service configuration will cover L15-7io, C9-4, and X7-2 probes. The period of performance is 11/15/2013 to 11/14/2014. Ultrasound Silver Service Agreements Includes: Priority scheduling for service calls and access to spare parts inventory, Priority Remote Service response (requires connection to Philips Remote Services network), All labor and travel during standard hours of coverage - Mon-Fri 8am - 5pm, excluding holidays, All parts (including transducers) that fail during normal use, Accidental Transducer Damage Protection: Coverage at 50% off the Philips Service Exchange Program price, Equipment Planned Maintenance services per Philips manufacturer specifications performed during standard hours of coverage, 35% discount off list price on the purchase of eligible system upgrades and Clinical Education tuition purchased with the upgrade. (not available for system platform exchanges), 35% discount off list price on the purchase of eligible transducers upgrades and Clinical Education tuition purchased with the upgrade, Proprietary operating system software enhancements without hardware changes (excludes software upgrades), Unlimited Technical Services & Clinical Applications Telephone Support, Remote Service, Utilization Reports, On-Board system diagnostics. The Government will award a fixed price contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (i)Technical capability of the item offered to meet the Government requirement (ii)Price (iii)Warranty The FOB point shall be destination. In order to be considered for an award, an offeror must have completed the online electronic Representations and Certifications located at https://orca.bpn.gov/ in accordance with FAR 4.1201(a). By submission of an offer, the offeror acknowledges the requirement that a prospective awardee shall be registered in the System for Award Management (SAM) at www.sam.gov/ prior to award, during performance, and through final payment of any contract, basic agreement, basic ordering agreement, or blanket purchasing agreement resulting from this solicitation [Note: Lack of registration in the System for Award Management (SAM) will make an offeror ineligible for award.] All responsible sources may submit an offer that will be considered by the Agency.
