Owner: US Department of Labor (DOL) Employment and Training Administration Center Operator: Adams and Associates, Inc This is a subcontracting opportunity with Adams & Associates, Inc. and NOT with the USDOL. Contract award will be based on quality, experience, expertise and price. Payment: Payment Terms are Net 30 days of completion and receipt of Invoice. Site Visit Date: To be scheduled. a) Contractor must visit the site and familiarize himself with the condition under which the work will be performed. No allowance will be made for any error on the contractors part for failure to take into account any conditions or circumstance existing at the site which might affect the contemplate work. The fact that a bid is submitted with ID # GL2014-016 will be taken as evidence that the contractor has full knowledge of the extent and character of the contemplate work and any problem to be encountered in the performing same. II. Project Description Scope of Project: To provide a driving instruction program for students in order to receive their NYS Drivers License. III Specific Requirements: Contractor: • Provided on-The Road Driver Instruction for Glenmont Job Corps Center Students in accordance with the Curriculum and requirements of the New York State Department of Motor Vehicles. • Ensure that instruction is in accordance with the requirement of the USDOL contracts, the Policy and Requirement Handbook (PRH), and centers standard operating procedures and handbook, utilization Job Corps Developed curriculum and program instruction and training achievement records. • Ensure that instruction incorporates a process of self-paced, open-entry/open-exit format learning and is structured in three modules: Module 1, Class Work; Module 2, Driving instruction; and Module 3, On-the-road practice. • Ensure that students complete a minimum of (20) hours for minors, recommended ten (10) hours for all others of behind-the-wheel practice and are, in the instructors opinion, competent to obtain driver to be scheduled for a road test. • Administer the required five (5) hour class. • Schedule and transport students to the Registry of Motor Vehicles testing center as appropriate. • Accurately maintain all required student progress, completion and attendance records, including, but no limiting to Progress Reports, Attendance and Accountability Reports, TARs and Student Employability Rating (SER) reports, as applicable. • Document and maintain the student's permanent record, including date of entry, date of completion of classroom training and date learner's permit was obtain, and provide a monthly report of all students participating in the program and their status. • Maintain a professional, safe instructional environment in the classroom, which includes standards of dress for both student and Subcontractor instructor and classroom appearance, organization and cleanliness. All required Center and OSHA safety standards, guidelines, polices and regulation shall be adhered to and maintained at all times. The Subcontractor will be responsible for the safety and security of any Academic provided equipment and facility. Any required repairs will be reported to the Academic Manager. • Provided a qualified instructor, or substitute instructor for classroom Instruction, or behind the wheel training on a year-round schedule, excluding Center-approved holidays and summer and winter breaks or as directed by the Center in the monthly schedule. The Center Academic Manager will provide direct oversight and supervision of this Subcontract, which will include evaluation of Subcontractor performance. • Ensure that the Driver Instructor Program is conducted eight times during the contract year. • Provide all instructional materials and supplies necessary to carry out the curriculum as required by the New York Department of Motor Vehicles and the Subcontractor. The Academy agrees to supply the appropriate classroom space and necessary equipment. Center: • Equipped classroom and Training facilities at the Glenmont Job Corps Center to be utilized by the instructor. • A Driver Instruction vehicle, Equipped with dual brakes, and prominently identified as a Driver Instruction vehicle. • Orientation for the instructor in all Glenmont Job Corps Center procedures and requirements • Staff support for the instructor in the administration, supervision and counseling of students. • By the 1st day of every month, a schedule of the Contractor showing the hours and days of the following month that the instructor is required to provide Driver's Instruction services. • Payment of all student fees for learner's Permits, Testing, and Driver's License. • All necessary documentation to support the instructor's role as a service provider at the Glenmont Job Corps Center. • A clear reporting process for the instructor to deal with administrative and student concerns and issues that arises in the context of the service. • Insurance coverage for all students undertaking Driver Instruction training and for all of the vehicles being used for that training. Note: Please check in at the welcome center on the scheduled days of service. IV Center Requirements Center Regulations: Contractor and his employees shall abide by all of the Center's rules and regulations. There shall be no fraternization between contractor personnel and Job Corps Center students or personnel. The Contractor will be required to follow all Center safety and security requirements. Management of Jobsite: The Contractor shall maintain a clean and safe workplace that is secured at the end of each working day. The Contractor shall clear all deliveries with the Center's security personnel. Insurance: The Contractor shall obtain and maintain insurance as follows: Workman's Compensation/Employers Liability insurance, to meet New York statutory requirements Comprehensive General Liability ($1,000,000 per occurrence) Comprehensive Automobile Liability ($1,000,000/combined single limit) Fire Legal Liability ($500,000 per occurrence) The Contractor must cover any deductible costs. Upon contract award, the Contractor must add Glenmont Job Corps Center as additional named insured to the policies. Schedule: From January 1, 2014 to December 31, 2014. Final Acceptance The Contractor shall notify the Center's facility manager when the project is substantially complete, and shall request an inspection. V. Proposal Form The Contractor's proposal must be submitted on company letterhead, must reference the proposal ID # GL2014-016 and address all items in sections II & III, to be considered. Bids must to be valid for 90 days. The Contractor shall submit the following information with the proposal: References Copies of Insurance Certificates and applicable licenses.
