- Request for Information (RFI) This is a Sources Sought Announcement and request for information only. This is not a Request for Proposals and does not constitute a commitment, implied or otherwise, that the Government will take procurement action on this matter. Requests for a solicitation will not receive a response. The Government will not be responsible for any cost incurred in furnishing information provided under this Sources Sought Announcement. Responses will assist the Government in determining the technical capability of potential responsible sources. This is sources sought notice issued in accordance with FAR part 5 and FAR Part 6.302-1. For information regarding this acquisition, contact the Contracting officials listed herein. All inquiries shall be in writing via e-mail. It is anticipated that this acquisition will be full and open competition, under NAICS 621340. 1. OVERVIEW The Defense Logistics Agency, DLA Contracting Services Office - Columbus is conducting a Sources Sought market survey to identify sources with the interest and capability to provide Occupational Health Services for the Defense Supply Center Columbus (DSCC) Installation. The services will be performed on post and the vendor must have a facility off post to perform services that cannot be performed on post within a 10 mile radius of the installation. It is requested sufficient information be provided in response to this RFI to assess the ability of industry to meet the service requirements with a facility located within a 10 mile radius of DSCC (3990 East Broad Street, Columbus, Ohio 43213). In addition to the above information requested, please provide a response time for submission of proposals. 2. INSTRUCTIONS TO POTENTIAL RESPONDENTS - DELIVERABLES This is NOT a Request for Proposal. Companies that desire to participate in this sources sought are encouraged to submit a response which supports the company's claim that it presently has the qualifications, experience, and capabilities to satisfy the requirements. The response should discuss the company's ability to provide the Occupational Health Services identified in the attached DRAFT solicitation. Potential responders should briefly discuss their experience in Occupational Health Contracts and state where their facility is located. Specifically, any interested companies are requested to provide the following information: 1) Name of Company 2) Business CAGE code 3) Point of contact and phone number 4) Size of Business according to North American Industry Classification System (NAICS) Code 621340 5) Summary of company's ability to perform Occupational Health work 6) Statement that contractor has a facility within a 10 mile radius of DSCC Installation 7) Number of calendar days necessary to provide a proposal in responce to a formal request for proposal. RESPONSES ARE DUE NO LATER THAN THURSDAY, 21 NOVEMBER 2013. The Government reserves the right to close this Sources Sought Notice once it has received an adequate number of responses.
