The following factors shall be used to evaluate offers: Technical Factors - Standard - - Acceptable - - Unacceptable 1. Provide all materials and professional services needed to perform required technical survey testing, professional continuing education training, and the evaluation of submitted test results for the continued accreditation of the Department of Pathology and Area Laboratory Services (DPALS) 2. Provide all analytical survey materials, to include: samples, electronic online surveys, result reporting forms or electronic reporting capability, and graded/evaluated survey results. 3. Delivery schedule for contracted test kits and online test surveys shall conform to the shipping dates established by the contractor for the calendar year. 4. The contractor shall grade/evaluate the submitted results and provide copies of the evaluation reports to the laboratory no later than 45 days after the suspense date for result submission 5. Evaluation reports shall include the values reported by the laboratory, identification of the method against which the results evaluation is based, and a graded comparison to the established target reference values for each tested analyte. The Government will use the following standards to evaluate Technical: (1) Acceptable - Proposal clearly meets the minimum requirements of the solicitation. (2) Unacceptable - Proposal does not clearly meet the minimum requirements of the solicitation. The Government intends to make an award without conducting discussions. After the initial evaluation of proposals is completed, if one or more proposals are rated Acceptable for both Technical and Past Performance, the initial evaluation will become the final evaluation of proposals and award will be made without discussions to the lowest priced offeror. If any technical subfactor is rated Unacceptable after the initial evaluation, the technical factor will be rated Unacceptable and the proposal will no longer be considered for award. If all proposals submitted result in an unacceptable technical evaluation then clarifications may be necessary. The Government evaluation team will evaluate the offeror's response to the Government exchanges with offerors.. (3)FACTOR : PRICE. Price will be evaluated for reasonableness based on anticipated adequate price competition. Once the technical and past performance factors are rated as acceptable, contract award will be made to the technically acceptable offeror with the lowest price. Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). A quotation is not an offer and, consequently, cannot be accepted by the Government to form a binding contract. Therefore, issuance by the Government of an order in response to a supplier's quotation does not establish a contract. The order is an offer by the Government to the supplier to buy certain supplies or services upon specified terms and conditions. A contract is established when the supplier accepts the offer. 52.212-3 OFFEROR REPRESENTATIONS AND CERTIFICATIONS--COMMERCIAL ITEMS (AUG 2013) ALTERNATE I (APR 2011)
