- SOURCES SOUGHT NOTICE This is a Sources Sought Notice only; it is not a request for quotations or offers. The Department of Veterans Affairs (VA) Tennessee Valley Healthcare System (TVHS) is conducting market research to identify potential sources for a contract to provide primary medical care services through a community-based outpatient clinic (CBOC) in: Warren County, McMinnville, Tennessee Christian County, Hopkinsville, Kentucky The period of performance shall be for a base period and four one-year option periods. For each requirement, there will be a single award made from the solicitation. The Government reserves the right to make no award from the solicitation. NAICS Code 621498 applies with a size standard of $10.0 M. Location Current Enrollment Warren County, McMinnville, TN1,603 Christian County, Hopkinsville, KY2,593 Under the authority of Public Law 104-262 and Title 38 U.S.C. 8153, the Contractor shall provide continuous delivery of Contractor Operation Primary Care Services, including Primary Care Mental Health and Preventive Medical Care Services in accordance with the terms, conditions, and provisions stated herein, and to furnish services to eligible veteran beneficiaries enrolled with the Tennessee Valley Healthcare System (hereinafter called TVHS). This solicitation covers the provision of Primary Care and Mental Health services. The primary clinic is to be located within the city limits of McMinnville, TN and Hopkinsville, KY. The contractor shall establish one location (for each) to provide the required services specified under this contract. Primary care and Mental Health must be provided at the same site as the primary clinic. -Veterans to be serviced under the McMinnville contract reside primarily in Cumberland, Loudon, Morgan, Rhea, and Warren in TN, but are not restricted to those counties. -Veterans to be serviced under the Hopkinsville contract reside primarily in Christian, Todd and Trigg counties in KY, but are not restricted to those counties. Primary Care and Mental Health services will be provided to patients enrolled to the clinic by the VA TVHS. Mental Health Services are anticipated for approximately 30% of patients enrolled. Payment for services will be based on a monthly capitated rate. Vendors interested in providing these services to the TVHS VAMC may contact Tina Smith via e-mail at Christina.Smith7@va.gov as well as annotating their interest in response to the FBO Sources Sought posting. Telephone responses will not be accepted. All interested parties should include the following information: 1. Company profile, including name of business and DUNS number. 2. Description of capabilities. 3. Demonstration of the firm's experience within the past five years, by listing the actual Outpatient Clinics operated as the prime contractor. Provide the following: a brief description of the work performed to include project title and a customer contact name and phone number for a customer knowledgeable of the work completed. 4. The business size and business socioeconomic category; such as Service-Disabled Veteran Owned Small Business, Veteran-Owned Small Business, Hub Zone Small Business, etc. Please note, all SDVOSBs and VOSBs must be verified by the office of Small Disadvantaged Business Utilization Center for Veterans Enterprise (CVE) prior to receiving any award. In accordance with Federal Acquisition Regulation policy, prospective vendors must be registered in Central Contractor Registration prior to award of contract. Interested parties should respond not later than December 6, 2013 at 4:00 P.M. Central Time. This Sources Sought Notice is to assist the Tennessee Valley Healthcare System in identifying an appropriate acquisition strategy only. Written communication received will be considered solely for the purpose of determining current market capabilities for the services identified within this notice. A solicitation is not currently available. If a solicitation is issued, it will be announced and posted on this website at a later date, and interested parties must respond to the solicitation to be considered for award. This notice does not commit the Government to contract for any supply or service. The Government will not pay for any information or administrative cost incurred in response to this Source Sought Notice. Do not contact any medical center staff in regards to this requirement, as they are not authorized discuss matters related to this procurement action.
