Attach 4-DRAFT-NREMT Price Schedule Attach 3-DRAFT-NREMT Evaluations Factors & Subfactors Attach 2-DRAFT-NREMT Agreement Attach 1-NREMT PWS dated 22 AUG 2013 THIS IS NOT A REQUEST FOR PROPOSALS OR A REQUEST FOR QUOTATIONS; IT IS STRICTLY A REQUEST FOR INFORMATION (RFI). NEITHER UNSOLICITED PROPOSALS NOR ANY OTHER KINDS OF OFFERS WILL BE CONSIDERED IN RESPONSE TO THIS RFI. NO CONTRACT WILL BE AWARDED PURSUANT TO THIS ANNOUNCEMENT. I. GENERAL INFORMATION: The Enterprise Sourcing Squadron (773rd ESS/PK) is seeking information concerning the availability of capable sources to provide two overlapping training sessions that result in certification of up to 60 Air Force EMTs to the Paramedic Level per year in accordance with the established standards set forth by NREMT, National EMS Education Standards and Paramedic Instructional Guidelines. II. DISCLAIMER: This RFI is issued solely for information and planning purposes and does not constitute a solicitation. Neither unsolicited proposals nor any other kind of offers will be considered in response to this RFI. Responses to this notice are not offers and will not be accepted by the Government to form a binding contract. Respondents are solely responsible for all expenses associated with responding to this RFI. All information received in response to this RFI marked as "proprietary" will be handled accordingly. Information provided as a response to the RFI will not be returned. The 773 ESS/PK will not entertain any questions concerning the composition, requirements, or the nature of services to be performed under any future request for proposal (RFP). III. REQUIREMENTS: In accordance with the Performance Work Statement (PWS) dated 22 Aug 2013 (Attachment #1), the contractor shall: Provide two overlapping training sessions that result in certification of up to 60 Air Force EMTs to the Paramedic Level per year in accordance with the established standards set forth by NREMT, National EMS Education Standards and Paramedic Instructional Guidelines. Each five month training session will consist of 25-30 students. In order to provide HQ AFMOA leadership oversight of this training, the training facility must be located within a 25 mile radius of Joint Base San Antonio (JBSA), Texas, due to the need for local Contracting Officer Representatives (CORs) oversight of the students. The contractor shall be fully certified and accredited by the Committee on Accreditation of Educational Programs for the Emergency Medical Services Professions (CoAEMSP), as granted by the Commission on Accreditation of Allied Health Education Programs (CAAHEP), as an approved program for paramedic training and certification. Required training shall include real world "ride along" (field internship) clinical program to all Paramedic students. Provide a training facility located within 50 miles of a Level 1 Trauma Center; a hospital equipped to handle any level of severity of trauma, and has a trauma surgeon on-site 24/7 and an Operating Room ready at all times for trauma cases as defined by the American College of Surgeons. Provide training that does not exceed 150 calendar days, including student travel. Training will consist of ALS (Advance Life Support) courses for Paramedic certification with NREMT to include Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS), and Basic Life Support (BLS). Provide all textbooks and an opportunity for each student who successfully completes the program to take the National Registry Paramedic psychomotor and cognitive examinations. Provide, at no cost, 1 NREMT Paramedic testing opportunity after graduation of the course but prior to student's departure back to home station. Provide, at no cost, remedial training for students following their first time failure of the NREMT Paramedic Exam. Remedial training days will be in addition to the 150 calendar days allowed for initial training. Re-testing must be scheduled and accomplished prior to the student departing; but will not exceed, under any circumstances, 179 calendar days. Provide all course material and any special uniform requirements to each student, (i.e., cargo pants, polo or tee-shirts required for clinical or ride-alongs). And provide all equipment, (i.e., stethoscope, trauma shears or utility scissors), and personal protective equipment for training, excluding pens, pencils, and writing paper. Provide certificates of completion to students upon successful completion of training. Provide students earned credit/semester hours with an official transcript at no additional cost or fee. A copy of the draft Educational Service Agreement (ESA) is contained in Attachment #2. Also, attached are the draft Evaluation Factors and Subfactors(Attachment #3), and Price Schedule (Attachment #4) IV. VENDOR RESPONSE If your firm is capable of providing the requirements described in this RFI, please provide a Capability Statement to Mr. Dustin Tatum, Contracting Specialist, via e-mail dustin.tatum@us.af.mil. Responses are due to this request for information (RFI) by 12:00 PM Central Standard Time (CST) on 25 Nov 2013. Please do not call the contracting office for information; email any questions to avoid any misunderstanding or confusion. The Capability Statement should be limited to 10 or less pages in length and include general information demonstrating how to meet the required objectives. At a minimum the following information is requested: 1) Company Information a. Company Name b. Address c. Point of Contact d. Telephone Number e. E-mail address 2) Commercial and Government Entity (CAGE) Code 3) Data Universal Numbering System (DUNS) Number 4) NAICS code(s) 5) Business Size (i.e. Small Business, Other than Small) 6) Socio-Economic status (i.e. 8(a), HUBZone, veteran-owned, women-owned, etc.) 7) A Rough Order of Magnitude (ROM) outlining the estimated costs to meet the requirements outlined in the PWS dated 22 AUG 2013. Include a per student estimate of tuition, fees, supplies, equipment, etc. 8) Breakdown of the training shown in PWS Para 2.1.6 9) Preliminary schedule for the training sessions, showing the start and end date in accordance with PWS Para 2.1.2 and 2.1.3. The response should describe the company's capability to offer and provide the training as addressed in this RFI. V. LATE SUBMISSIONS: Failure to respond to this RFI does not preclude participation in any future competition, nor will information provided in response to this RFI be used to exclude anyone from responding to any future requests for proposals. Communications with 773 ESS/PK with regard to this RFI will only be permitted in writing (via fax or email) during the RFI response period. Responses to the RFI received by 773 ESS/PK after the submittal deadline and time indicated may be considered. The Respondent assumes the risk of the method of dispatch chosen. Postmarking by the submittal date and time shall not substitute for actual response receipt. VI. OWNERSHIP OF RESPONSE TO RFI: All informational material submitted in response to this request become property of 773 ESS/PK. VII. RELEASE OF CLAIMS, LIABILITY, AND PREPARATION EXPENSES: Under no circumstances shall 773 ESS/PK be responsible for any response preparation expenses, submission costs, or any other expenses, costs, or damages, of whatever nature incurred as a result of the Respondent's participation in this RFI process. Respondent understands and agrees that it submits its response at its own risk and expense, and releases 773 ESS/PK from any claim for damages or other liability arising out of the RFI process. VIII. ERRORS IN RESPONSE: 773 ESS/PK shall not be liable for any errors in Respondent's response. Respondent is responsible for careful review of its entire response to ensure that all information is correct and complete. Respondents are liable for all error or omissions contained in their responses. IX. ADDENDUM: 773 ESS/PK reserves the right to issue an addendum to this RFI at any time for any reason.
