Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in far subpart 12.6-streamlined procedures for commercial items, in conjunction with far subpart 13 and as supplemented with additional information included in this notice. This is a Firm Fixed Priced Contract. Only qualified offerors may submit bids. Solicitation VA256-14-R-0149 is issued as a Request for Proposal (RFP) and constitutes the only solicitation, a written solicitation will not be issued. This solicitation is set-aside for SDVOSBs. If claiming SDVOSB, firm must be registered and CVE verified in VetBiz Registry www.vetbiz.gov. NAICS Code is 541350 and size standard is $7 Million. Any firm that does not meet the capability and size standard under this NAICS code should not submit a response to this notice. In order to be considered for a government award, the firm must be registered in System for Award Management @ www.sam.gov and be capable of furnishing all labor, parts, material, travel/shipping to provide the following services listed in statement of work. A. STATEMENT OF WORK: To update The Joint Commission Statement of Conditions (SOC) for the Jack C Montgomery VA Medical Center (JCMVAMC), Muskogee, OK Buildings 1 and 53 and Ernest Childers Out Patient Clinic (TOPC). Tulsa, OK. The scope of services will include the following: 1. Conduct a survey of the Buildings in order to determine compliance with both the 2000 and 2012 editions of NFPA 101, Life Safety Code. 2. Provide recommendations for the appropriate occupancy classification for each section of each building. 3. Evaluate the existing fire and smoke barrier walls and make recommendations to decommission the walls and cross corridor doors that are no longer required. 4. Conduct a random (10%) survey of fire and smoke barrier walls above the accessible ceilings to confirm location and construction. On redlined drawings, denote wall deficiencies noted for future repair. 5. Identified and documented deviations from NFPA 101 so that maintenance related work orders and Plan(s) for Improvement (PFI) can be developed. 6. Review our electronic Joint Commission data and assist in its update. 7. The following data shall be presented in a format using Microsoft Word. A. Statement of Qualifications. B. General building description including height, size, construction classification and occupancy. C. Information provided by the medical center regarding any equivalencies approved by The Joint Commission (if any). D. Life Safety Assessments. E. Small scale floor plan drawings showing the current room layout, fire walls, smoke barrier walls, smoke compartment names or designations and occupancy classification for each floor, also identify suites, hazardous rooms and key note any special life safety features. F. Make recommendations on how to correct any noncompliant items that are not correctable by simple, straight forward methods. G. Prepare a preliminary Plan for Improvement in the draft submittal for any items that is not correctable under our work order system. After receiving the medical center's input, complete the PFI for inclusion in the final SOC documents. 8. All minor deficiencies that are best corrected through normal maintenance efforts or work orders shall be provided by separate letters and lists. These deficiencies will only be included in the draft SOC documents. 9. Three draft copies of the SOC documents will be submitted for review. 10. Provide follow-up technical support by telephone/electronic mail to your staff on the 2013 Statement of Condition or other fire related issues leading up to the next Joint Commission survey for a total of 12 hours anticipating an average of 1 hour a month. 11. Provide one master and two numbered copies of the SOC documents and a computer CD containing the final SOC documents and drawings. 12. Update electronic Joint Commission data to reflect final SOC document conditions (Provided "eSOC access only" status is granted by the Medical Center). 13. Provide an exit conference after all surveys are completed with Muskogee, Oklahoma VAMC Safety, Engineering, and Leadership staff to discuss findings and trends for a total of two hours. B.Applicable Documents: The Joint Commission Manual C.Contractor Requirements Delivery is required within 180 days. D.Government Requirements The hospital will provide a CD containing the most current electronic drawing files (in AutoCAD) and current fire and smoke barrier wall arrangements for review and re-plotting prior to our site survey. Access to the necessary keys to access all areas, a full time escort or general engineering keys and an escort for two hours each day so access and survey of all areas of the building can be conducted. Completion of draft documentation within 30 days of site survey will be required. Final documentation will be prepared within two weeks after receipt of the final review comments. E.Deliverables: One master and two numbered copies of the SOC documents and a computer CD containing the final SOC documents and drawings. This is a competitive Request for Proposal (RFP) that represents the best value to the Government conducted under FAR Part 15, Lowest Price Technically Acceptable (LPTA) source selection process. Award will be made on the basis of the lowest evaluated price of proposals meeting or exceeding the acceptability standards for non-cost factors. Proposals will be evaluated and ranked according to price, from lowest to highest. The Government reserves the right to evaluate only the technical proposal of the lowest price offeror. Other technical proposals may not be evaluated unless the Government determines the lowest price offeror to be unacceptable or non-responsive. In the event the Government determines the lowest price offeror to be unacceptable, the technical proposal of the next lowest price offeror will be evaluated. This process will continue until a technical proposal is determined to be acceptable. The lowest offeror deemed acceptable in all non-cost factors shall be the apparent awardee. Price proposals will be evaluated and ranked according to price, from lowest to highest. Price will not be evaluated other than to ensure price reasonableness. The Government may use various price analysis techniques and procedures to make a price reasonableness determination. Note: Offers that are not technically acceptable cannot be selected, regardless of Price. The Technical evaluation factors consist of two separate elements: Experience and Technical proposals will be rated either acceptable or unacceptable. The offeror needs to show that they have technical expertise to perform services listed in statement of work. Offers that do not provide enough detail experience will be rated as unacceptable. **SITE VISIT INFORMATION: Site Visit will be held at the Jack C Montgomery VA Medical Center (JCMVAMC), Muskogee, OK on November 26, 2013 at 10:00 CST. Please meet at the Safety Office located behind the main hospital. Please provide all questions in writing within 48 hours of site visit.
