Description
This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. Responses to this Sources Sought must be in writing. This RFI is a follow on to RFI VA262-13-I-0072 and VA262-13-I-0072_1. The purpose of this Sources Sought Announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified Small Businesses interested and capable of performing the work. Responses to this notice shall include evidence of recent (past three years) past performance performing the same or very similar services as identified in the attached excerpt from the draft Statement of Work. It is emphasized that this contract will require the Contractor to manage the shift schedule and provide all physicians to cover all shifts in this level II Emergency Department which operates 24 hours per day and 365 days per year. Some detail of the requirement is provided below in an excerpt from the draft Performance Work Statement. Contractors who are small businesses only should respond to this RFI by submitting the following not later than Tuesday, November 26, 2013 via email to Sandra.Fusco@va.gov: (a) company name b) address (c) point of contact (d) phone, fax, and email (e) DUNS number (f) Type of small business, if applicable, (e.g. Services Disabled Veteran Owned small Business, Veteran-owned small business, 8(a), HUB-Zone, Women Owned Small Business, Small disadvantaged business, or Small Business) and (g) a capability statement that addresses the organization's qualifications and ability to perform as a contractor for the work described below. In addition, Contractors will include evidence of successful past performance performing the same or very similar services by including the contract name, contract number and points of contact (email and phone) for this performance. All Offerors who provide goods or services to the United States Federal Government must be registered in the System for Award Management (SAM) database found at https://www.sam.gov. Registration must include Representations and Certifications. Additionally, all Service Disabled Veteran Owned Businesses or Veteran Owned Businesses who respond to a solicitation on this project must be registered and verified with the Vet-Biz Registry located at http://vip.vetbiz.gov. It is the VA's intention to solicit proposals on a competitive basis for a Firm-Fixed Price contract. Proposed prices shall be all inclusive and will include physician fees, malpractice insurance, benefits and contractor's fees. The contract will be awarded as a base year (one year from date of award), with four (4) one-year renewal options that can be exercised at the option of the Government. This solicitation will be issued as a commercial item in accordance with FAR Part 12 and FAR Part 15. NAICS is 621111. The solicitation is expected to be issued on or about March 1, 2014 with responses due 30 days after release. Point of contact for questions and responses is Sandra Fusco at Sandra.Fusco@va.gov. The following excerpt from the DRAFT Performance Work Statement is provided: 1. BACKGROUND. a. The VA Long Beach Healthcare System (VALBHS) has an impending requirement for the provision of Emergency Department (ED) Physician and Nurse Practitioner services for VA beneficiaries. VALBHS serves a Veteran/patient population of approximately 50,000 Veterans in the greater Long Beach area and is defined as a Level II facility due to the inpatient acute care, ICU, and surgical complexity capabilities. b. Key elements of a Level II Trauma Center include 24-hour immediate coverage by Emergency Department Physicians 24 hours per day, 7 days per week, 365 days per year. It is a facility that is able to initiate definitive care for all injured patients. c. The Contractor shall provide six (6) full time equivalent (FTE) Emergency Medicine (EM) Physicians and 1.5 FTE Nurse Practitioners (NPs) to cover the range of services normally found in a similar civilian healthcare facility. Additionally, the Contractor shall identify one (1) lead physician to perform both clinical and management/admin duties as described herein. d. Patient load is estimated to be approximately 85 patients per day, but may be higher or lower on certain days. d. The ED is made up of (16) private exam Departments and (1) triage Department. 2. GENERAL REQUIREMENTS. a. The Contractor shall provide EM board certified physicians to adequately cover all shifts, 24 hours a day, 7 days a week, 365 days per year. A provision for an overlap of up to one hour at the end of each shift is incorporated and is projected to be utilized to provide overlapping coverage during shift changeover in order to promote a smooth transition in services provided. b. Contract Physicians shall not work more than sixteen (16) continuous hours per day nor exceed eighty (80) hours of duty per calendar week (Sunday-Saturday). c. There shall be at least one ED Physician during each shift. d. The requirement for Nurse Practitioner coverage is estimated at 10 hours per day during peak hours, as scheduled by the VA, 7 days per week. e. Emergency Department Physicians will not be required to provide care in areas other than in the Emergency Department (with the exception of endotrachel intubations and airway management), unless there is an unusual emergency situation or a disaster. Physicians will be required to participate in endotracheal intubations and airway management of emergent and semi-emergent patients. f. VA personnel required for ED services at VALBHS will be provided by the VA at levels mutually agreed upon and compatible with the safety of the patient and personnel and with quality medical care programming. g. The services to be performed by the Contractor shall be performed in accordance with VA policies, procedures, and the regulations of the medical staff by laws of VALBHS facility. Contractor shall meet or exceed VA and Joint Commission standards. 3. REQUIREMENTS FOR EVALUATION AND TREATMENT OF PATIENTS. a. Contract Physicians and Nurse Practitioners shall evaluate and provide treatment for VA patients who are transferred from other facilities, transported via ambulance or private conveyance, escorted in, or who walk in for care. Specific requirements include the following: (1) All patients presented to the ED shall be seen in a timely manner. Timeliness will be defined based on criticality of the patient, and triage category, not time of their arrival. However, all efforts shall be put forth to manager patient needs in 6 hours or less. (2) Develop a plan of care for a patient according to the triage category and presenting condition, and the Medical Screening Exam (MSE). (3) Establish plans for follow-up care with outside agencies or Primary Care manager as appropriate. (4) Prescribe medications for patients according to appropriate clinical protocols and the established drug formulary within the VA Long Beach Medical Center. (5) Establish plan for hospitalization (as applicable) through coordination with inpatient units and VA staff physicians. Additionally, have an understanding and be prepared to transfer patients to surrounding community facilities if bed space or necessary services are unavailable to meet patient care needs. (6) Assist in the transfer of patients to local facilities for necessary treatment and services unavailable at the Long Beach campus pursuant to VALBHS policies and procedures. (7) Effectively communicate and document a plan of care with patient, care provider, and family. The contractor shall submit regular reports, (i.e. weekly, monthly, or any mutually agreed upon time frame with the VA Medical Center) indicating this has been accomplished, assuring quality of care. (8) Provider shall complete an electronic encounter form and a progress note at the end of the patient encounter for accurate collection of data, workload, and billing purposes. The provider shall accomplish this solely through Computerized Patient Record System (CPRS) in Veteran's Health Information Systems and Technology Architecture (VISTA). Choices shall be made based on current ICD-9 and CPT coding guidelines and shall be supported by complete provider documentation. c. The VA will provide the following: (1) All equipment and supplies necessary to manage and treat illness/injuries presented to the Emergency Department. Contract staff may not bring personal equipment to the department without prior written approval of the facility. NOTE: Personal stethoscopes are an exception to this rule. (2) Laboratory services for evaluation and processing. The Long Beach VA will be responsible for submitting timely reports to the requesting physician. (3) Radiology/CT/MRI Services for evaluation and processing. The Long Beach VA Radiology Department will be responsible for submitting timely reports to the requesting physician. (4) Pharmacy Services will provide all necessary medications for temporary outpatient treatment (3 days). Any long term needs will be prescribed once a care plan is accomplished by the patient's Primary Care Manager. (5) VALBHS will provide adequate and appropriate nurse staffing personnel at all times. 4. QUALIFICATIONS. Personnel assigned by the Contractor to perform services covered by this contract shall hold an unrestricted license in a State, Territory, or Commonwealth of the United States or the District of Columbia. The qualifications of such personnel shall be subject to review by the VALBHS COS and approval by the VALBHS Director. a. Emergency Medicine Physicians shall: (1) Be Board Certified in Emergency Medicine by the American Board of Emergency Medicine. (2) Have completed an Emergency Medicine Residency Program. (3) Be credentialed by the appropriate civilian American Medical Association specialty and indemnified by an appropriate/legal third party corporate entity for their professional activities. (4) Have a minimum of two (2) years of experience in emergency Department care within the last three (3) years. Time spent in an emergency Department setting as a physician intern or in a residency program shall count toward the two (2) years of experience. VA experience preferred. (5) Be currently certified in American Heart Association Basic Life Support and Advanced Cardiac Life Support. (6) Be a U.S. citizen. (7) Not have had a medical license revoked or limited by a State, Territory, Commonwealth or the District of Columbia or be pending any such action. (8) Not be pending or have had past felony charges against him/her. b. Nurse Practitioners shall: (1) Be licensed or otherwise recognized as a nurse practitioner in a State, possess a master's degree from a program accredited by the National League of Nursing Accrediting Commission (NLNAC) or Commission on Collegiate Nursing Education (CCNE), and maintain full and current certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body. The certification must be in the specialty to which the individual is being appointed or selected. Authorization for prescriptive authority is required. (2) Have three (3) years of clinical nursing experience, with a minimum of one (1) year clinical experience as a Certified Registered Nurse Practitioner (CRNP), three (3) years preferred. Experience in outpatient care in Emergency Department environment is also required. (3) Be currently certified in American Heart Association Basic Life Support and Advanced Cardiac Life Support. (4) Possess current Emergency Department Certification (5) Meet all of the credentialing and privileging criteria and complete this process before practicing at VALBHS (6) Be a U.S. Citizen 5. Lead ED Physician. The Contractor will identify one (1) lead ED physician who shall provide both clinical and administrative support of the contract and report to the Chief of the Medicine and Inpatient HCG. The clinical support shall be no less than 80% of the contract requirement for one FTE. All clinical qualifications and duties of the ED Physician as stated here-in apply to the lead ED Physician as well. Duties required of the lead ED Physician include: (1) Serves as a subject matter expert on medical center committees in a non-voting capacity (2) Serves as a subject matter expert working with the Chief of Medicine and Inpatient Healthcare Group to improve ED flow of operations (3)Coordinates with COR to ensure contractor employee orientation for all CPRS training, certificate mandates/access, PACS user system Radiology, entering expedite lab results (4) Attends quarterly training to maintain and train team on emergency contingency record keeping in case of a power loss (5) Serves on the MEC by the Chief of Staff as an "AD HOC" member (6) Attends Resuscitation Service Committee meetings during Administration hours at the medical center. (7) Manage the scheduling of Contractor personnel to ensure that the coverage requirements of the ED are met as stated herein and directed by the Chief of Medicine Service.
