Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation is attached. This solicitation is being conducted under FAR 13 Commercial Items as a Request for Quote (RFQ) No. VA251-14-Q-0095. This solicitation closes on November 22, 2013 at 12pm EST. The Network 11 Contracting Office is soliciting quotes from contractors to provide Anesthesia Equipment Maintenance Services to the Northern Indiana Health Care System located at 2121 Lake Street, Ft. Wayne, Indiana. A Base plus four (4) Option Year Firm Fixed Price Contract is being contemplated. Interested contractors will be responsible for downloading copies of solicitation (and any subsequent amendments) from the Federal Business Opportunities website (www.fbo.gov), no hard copies will be made available. The solicitation will provide full information to contractors about the quote submission process. NAICS Code is 811219, and Size Standard is $19 Million. The solicitation will be issued as a Small Business Set Aside. All responsible sources may submit proposals which shall be considered by the Agency. To be eligible for award, contractors must be registered in the SAM (System for Award Management) data base at https://www.sam.gov/portal/public/SAM/. Registration is free and potential offerors are encouraged to visit that SAM website and register as soon as possible to avoid delays should they be selected for a contract award.
