- Solicitation CSOSA-14-R-0100 FOR DNA SAMPLING PLUS ATTACHMENTS + LETTER Combined Synopsis Solicitation CSOSA-14-R-0001 (Reissued as CSOSA-14-R-0100 for FedBizOps) This is a solicitation for proposals/quotations. This synopsis does constitute a Request for Proposal (RFP) or Invitation for Bid (IFB). The Court Services and Offenders Supervision Agency (CSOSA) for the District of Columbia is seeking vendors capable of delivering DNA Sampling and Tuberculosis (TB) Testing Services. The TB Serum may be purchased via a separate contract vehicle. The contractor will be required to provide services that will include but is not limited to: logging in testing, taking blood samples, and administering tests. Reliable administration and reading of the Tuberculosis test requires standardization of procedures, training, supervision and practice. These services are performed at one location within the District of Columbia, at 300 Indiana Ave., NW, Washington, D.C. The North American Industry Classification System (NAICS) code is 561311 - Employment & Placement Agencies, as well as 621399 Miscellaneous Health Practitioners. This solicitation is open to all sources. This solicitation will result in a firm-fixed price contract. The performance period is one (1) base year and four (4) one year option periods. The Option Periods are exercised at the preference of the Government. This solicitation may result in a single or multiple award contracts, and the Government reserves the right to award CLINs separately. The place of performance for the resulting contract will be in the District of Columbia. Questions regarding this Request for Proposal (RFP) must be submitted in writing via e-mail or fax to the contracting officer identified for this solicitation no later than December 10, 2013 at 11:00 A.M. Eastern Standard Time (EST). You may email questions to Moira.McLaughlin@csosa.gov before December 10, 2013. Offerors must submit their proposal by January 30, 2014 at 11:00 A.M. EST (local). Proposals received after this date and time will be considered late and will not be considered for award. The Government requests that proposals be valid for a period of up to 120 days after submission. All interested parties are responsible for monitoring the FedBizOpps website for release of solicitation updates, amendments and changes to the solicitation. No paper copies of the solicitation will be issued. Prospective Offerors must be registered in Central Contractor Registration (CCR) to be eligible for a contract award. Registration requires applicants to have a Data Universal Numbering System (DUNS) provided by Dunn & Bradstreet (D&B) and a Tax Identification Numbers (TIN).
