- Statement of Requirements OFFADD: 42 CONS/LGCA 50 LeMay Plaza South Montgomery, AL 36112 SUBJECT: Wireless/Cordless Dental Loupes W/ Lights DUE: 29 November 2013 POC: Cory Rasnic, Contract Specialist, Phone: (334) 953-3528; Fax (334) 953-6170 DESC: The 42nd Contracting Squadron at Maxwell Air Force Base is requesting information for Cordless/Wireless Dental Loupes. This is a Request for Information (RFI) ONLY. It is not a Request for Proposal (or solicitation) or an indication a contractual commitment will exist for this requirement. THE GOVERNMENT DOES NOT INTEND TO AWARD A CONTRACT ON THE BASIS OF THIS RFI OR OTHERWISE PAY FOR THE INFORMATION SOLICITED. The purpose of this market research (Source Sought) is to determine if the Wireless/Cordless Dental Loupes W/ Lights below are commercially available. Wireless/Cordless Dental Loupes W/ Lights should be brand name or equal of the following items: CLIN 0001: XV1 - HiRes CLASS 3 (QTY 3 Each) CLIN 0002: HiRes Class 2 Loupe (Sport Frame) W/ Anti-Reflective Coating(QTY 1 Each) Firms responding to this RFI are asked to provide the following information in their initial response: company's full name, business size, address, point of contact, and a clear indication as to whether or not they can provide the items the government is seeking. Final responses to this RFI will be due at 4pm, Central Standard Time (CST), on 29 November 13. As industry may expect to see portions of submitted information in future solicitations, portions of the response containing proprietary information should be clearly marked as such. Information submitted will not be returned to the respective respondents. Oral submissions of information are not acceptable and will not be accepted in response to this notice.
