- Attachment 2 - DRAFT Performance Work Statement Attachment 1 - Capabilities Statement Cover Sheet Sources Sought Synopsis - Medical Patient Transporter SOURCES SOUGHT SYNOPSIS (Not a Notice of Solicitation) This is a Sources Sought Synopsis; there is no solicitation available. Requests for a solicitation will not receive a response. This Sources Sought Synopsis is published for market research purposes. This market research will be used to help determine if this may be a competitive acquisition, or if a set-aside for any small business program is appropriate, sources including small business (SB), 8(a), Hubzone, Women Owned Small Business (WOSB) and Service-Disabled Veteran-Owned small business (SDVOSB). Please review this announcement, including all attachments, in their entirety. Request that prospective small business offerors complete and return a Capabilities Statement as described below and at Attachment 1 to identify firms who possess both the capability and experience to provide the item described in ATTACHMENT 2. The Government is in no way obligated to do business with or to enter into any form of contract with any person, firm or other entity that receives or responds to this announcement. The Operational Contracting Branch, Technology Support Section at Wright-Patterson AFB, Ohio anticipates issuing a Commercial Request for Proposals for Medical Patient Transporter and is seeking potential sources, including small businesses. The Government anticipates a competitive acquisition for this award. The contractor shall provide all personnel and materials needed to provide Medical Patient Transporter in accordance with the INFORMATION IN ATTACHMENT 2, Performance Work Statement, included in this posting. In addition to the information requested in the paragraph below, entitled "Capabilities Statement," responding parties must also indicate their size status in relation to the applicable North American Industry Classification System (NAICS). The NAICS Code is 621399 with a Size Standard of $7 Million. Firms responding should indicate their size in relation to this size standard, and indicate socio-economic status (SB, 8(a), SDVO, WOSB, and Hubzone). The FSC Code for this acquisition is Q999. Respondents are further requested to indicate their status as a Foreign-owned/foreign-controlled firm and any contemplated use of foreign national employees on this effort. The Government reserves the right to consider a small business set-aside based upon responses hereto for any subsequent acquisition. CAPABILITIES STATEMENT Interested parties must submit a Capabilities Statement, with the cover sheet provided completed, that is brief and concise, yet clearly demonstrates ability to meet the stated requirements to the Point of Contact listed below. The Capabilities Statement should clearly present evidence that the interested party is fully capable of providing the required service and as such may contain any information that the interested party feels is relevant. A maximum of 4 pages may be submitted, not including the cover sheet. Capabilities Statements are to be presented in accordance with the attachment to this sources sought synopsis entitled "CAPABILITIES STATEMENT." Interested parties are also encouraged to provide feedback regarding the INFORMATION IN ATTACHMENT 2, including a list of any additional information that they may require in order to provide a proposal for this acquisition. (The Government may not be able to provide all of the information that is requested.) Any information submitted by respondents to this sources sought synopsis is voluntary. This sources sought notice is not to be construed as a commitment by the Government, nor will the Government reimburse any costs associated with the submission of information in response to this notice. Respondents will not be individually notified of the results of any government assessments. The Government's evaluation of the capability statements received will factor into whether any forthcoming solicitation will be conducted as a full and open competition or as a set-aside for small businesses, or any particular small business designation (e.g. SDVOSB, HUBZone, 8(a), etc.).
