Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in far subpart 12.6-streamlined procedures for commercial items, far subpart 13.5-test programs for certain commercial items, and as supplemented with additional information included in this notice. This is a Firm Fixed Priced Contract. Only qualified offerors may submit bids. Solicitation VA-249-14-Q-0064 is issued as a Request for Quotation (RFQ) and constitutes the only solicitation, as written solicitation will not be issued. This solicitation is issued as full and open competition under NAICS Code 811310 and size standard $7 Million. In order to be considered for a government award, the firm must be registered in SAM @ www.sam.gov and be capable of providing all necessary travel, labor, materials, tool documentation, and parts required for the inspection, support of the equipment, software, and accessories listed in the Schedule of Equipment to successfully complete preventive maintenance and operational coverage on the York Chillers, Cooling Towers, Water Coolers and any additional equipment listed in the attached statement of work. This service is required for the VA Medical Center, l 030 Jefferson Avenue, Memphis, TN 38104, in accordance with the terms, conditions, special provisions and schedules herein. In order to be responsive, the firm must offer a quote on all itemized items and total base year including all option years pricing as provided below. Base Year: 12/16/2013 - 9/30/2014 Option year l: 10/1/2014 - 9/30/2015 Option year 2: 10/1/2015 - 9/30/2016 Option year 3: 10/1/2016 - 9/30/2017 Option year 4: 10/1/2017 - 9/30/2018 SERVICE REQUIRED LOCATION BASE YEAR Pricing OPTION YEAR l Pricing OPTION YEAR 2 Pricing OPTION YEAR 3 Pricing OPTION YEAR 4 Pricing Premium Planned Service and inspection services in accordance with the statement of work (SOW) for eight (8) chillers, and five (5) cooling towers.
