STATEMENT OF OBJECTIVES B.2 STATEMENT OF OBJECTIVE (SOO): SCOPE: Removal, transporting and disposal of regulated medical and pharmaceutical waste at the following VA health care centers in Oklahoma in accordance with local, state and/or federal requirements: "Jack C. Montgomery VA Medical Center, 1011 Honor Heights Drive, Muskogee OK 74401 "Ernest Childers VA Outpatients Clinic, 9322 East 41st, Tulsa OK "Tulsa Behavioral Medicine SVC, 10159 E. 11th Street, Tulsa OK 74128 Type of Contract: Firm Fixed-Price Period of Performance: Base Period + 4 (1) year options Regulated Medical and Pharmacy waste defined: Regulated medical waste is defined by the Oklahoma Department of Environmental Quality as a waste or reusable material that contains an "etiologic agent" and is generated in the diagnosis, treatment or immunization of human beings or animals; research pertaining to the diagnosis, treatment or immunization of human beings or disabling or fatal disease. The term "etiologic agent" is synonymous with the term "infectious substance". Examples of RMW include: "cultures and stocks of etiologic agents or live vaccines; "human blood, blood products, and human body fluids, except urine or feces; "pathological wastes consisting of human tissues, organs, and body parts removed during surgery, autopsy, biopsy and other medical procedures; "untreated sharps; "used blood collection bags, tubes, and vials; "contaminated carcasses, body parts and bedding of animals intentionally exposed to pathogens in research, in the production of biologicals or the "in vivo" testing of pharmaceuticals; "items contaminated with blood or other human body fluids which drip freely or would release such materials in a liquid or semi-liquid state if compressed or are caked with dried blood or body fluids and are capable of releasing these materials; "isolation wastes unless determined to be noninfectious by the infection control committee at the health care facility; "HIV-containing cell or tissue cultures, organ cultures, and HIV- or HBV-containing culture medium or other solutions; and blood, organs, or other tissues from experimental animals infected with HIV or HBV; "All disposable materials that have come in contact with cytotoxic or antineoplastic agents during the preparation, handling, and administration of such agents. Such wastes include, but are not limited to, masks, gloves, gowns, empty IV tubing and bags, vials, and other contaminated materials; and "any other material or equipment which, in the determination of the health care facility staff, infection control committee or other responsible party, presents a significant danger of infection because it is contaminated with, or may reasonably be expected to be contaminated with, etiologic agents. In the event of an emergency, contractor services may be stopped and rescheduled at no additional cost to the government. Holidays for all facilities: "New Year's Day (January 1). B.2.2 Contractor Requirements: Contractor shall provide a performance work statement (PWS) that will outline how their company will manage the removal, transporting and disposal of regulated medical and pharmacy waste at all facilities governed by local, state and federal laws for regulate medical and pharmaceutical waste under the given constraints outline in Section B.2.1 The contractor shall also provide a quality assurance plan (QASP) to charting how the performance objective will be met as stated under Section B.4, to include contingency plans of operation. The Contractor shall furnish the means of transporting and all the equipment, supplies required to perform the services under this contract. The contractor shall provide timely, accurate and clear manifests or report documents of weight of each type waste picked up at each of the three locations, including permits for incinerator, autoclave or any other legal waste technology and transportation. The manifest must meet all federal and state requirements. The manifest shall be provide to the COR (Contracting Officer Representative) The Contractor shall furnish state/federal approved containers to collect regulate medical waste and pharmacy waste use in designate areas, at the (3) three medical health care centers listed in Section (B.2.1). Contractor must certify and be properly licensed by Federal, State and Local authorities to provide pickup, removal and disposal of regulate medical and pharmaceutical waste. Contractor must attach copies of licenses, permits and certification at time of proposal due date. The contractor will complete a general background check on all personnel assigned to this service contract and provide a copy of report to COR prior to starting the service. The contractor staff assigned for pickup, removal and transporting of regulate medical and pharmaceutical waste will need be identifiable with the company they are working for. The contractor will ensure that employees assigned to this service contract, possess basic proficiency in speaking and writing English. The Contractor staff responsible for pickup, removal and transporting of regulate medical and pharmaceutical waste shall promptly notify the COR and Safety Officer of any spills occurring during pickup and loading of contractor truck and shall be responsible for cleanup of spills caused by contractor staff in close coordination with facility Safety Officer.
