Description
Combined Synopsis/Solicitation for Laboratory Accreditation Services Description: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is RFQ-DC-14-00018 and is being issued as a Request for Quotations (RFQ), in conjunction with FAR 13 Simplified Acquisition Procedures. The solicitation document and incorporated provisions are those in effect through FAC 2005-70. The applicable North American Industry Classification System (NAICS) Code for this procurement is 813920. This procurement is being conducted as a full and open competition. The United States Environmental Protection Agency (US EPA), Office of Chemical Safety and Pollution Prevention (OCSPP), Office of Pesticide Programs (OPP) has a requirement for a vendor to provide laboratory accreditation services for the ISO/IEC 17025 standard accreditation of all three (3) OPP Laboratories, in compliance with EPA laboratory policy. The work performed by the contractor shall consist of tasks and services necessary for the OPP laboratories to achieve accreditation to the ISO/IEC 17025 standard. For each of the three OPP laboratories, the contractor will be the accreditation body and shall: -Develop an individual laboratory accreditation plan and propose assessment team members for each laboratory. -Conduct the accreditation assessment for each laboratory -Prepare and submit a final written report addressing the results of each laboratory?s assessment. -Review and respond to each laboratory?s responses and corrective actions to the findings in the assess ment report. -Grant accreditation. -Be responsible for performing all accreditation body activities necessary to maintain each laboratorys accreditation. Provide, during the accreditation cycle, review and assessment plans, document reviews, on-site assessments, report preparation, and all other activities that are required by the accreditation bodys accreditation cycle. For more detailed information about this requirement please refer to the Statement of Work (SOW) and Evaluation Factors, which are accessible at: http://www.epa.gov/oamhpod1/oppts_grp/14jg001/index.htm. EPA anticipates awarding a firm fixed-price purchase order. The period of performance will be from the date of award to completion of the final accreditation cycle. While the accreditation cycle length and requirements will be defined by the accreditation bodys policies and procedures, a two year cycle length has been used for planning this effort. Basis for Award: Quotes submitted in response to this RFQ will be evaluated on a lowest evaluated price/technically acceptable basis. The evaluation factors are provided at the link listed above. Instructions: FAR 52.212-1 'Instructions to Offerors - Commercial' applies to this acquisition. Offerors are to include a copy of FAR provision 52.212-3 'Offeror Representations and Certifications - Commercial Items' with their response. The following FAR clauses also apply: 52.212-4 'Contract Terms and Conditions - Commercial Items' and 52.212-5 'Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items'. The aforementioned provisions and clauses can be accessed electronically at http://www.acquisition.gov/far/. Prospective sources are reminded that award can only be made to a contractor who is registered in the System for Award Management (www.sam.gov) database with an active Dun & Bradstreet Number. Partial quotes will not be accepted for this requirement. In addition, the FAR 52.212-3 representations and certifications MUST be submitted if they are not already completed and available online at sam.gov. Quotes shall be submitted via e-mail in Word or Adobe to Jennifer Garger, Contract Specialist, at garger.jennifer@epa.gov no later than 2:00 p.m. EST on Friday, December 13, 2013.
