Description

This is a combined synopsis/solicitation for the purchase of commercial services, prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. Solicitation number VA260-14-Q-0138 is applicable and is issued as a request for quotations. The solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-70. The relevant NAICS Code is 485991 with a corresponding size standard of $7M dollars. Offerors shall submit price offers for base year and all options to angela.williams3@va.gov. This acquisition is a Total small business set-aside. Contractor shall perform Cabulance Wheelchair lift & Gurney Services for the VASORCC/ VA Southern Oregon Rehabilitation Center & Clinics, 84965 Crater Lake Hwy., White City, OR 97503. Period of Performance Date of Award to 09/30/2018 plus Four option Years CLINS 0001/1001/2001/3001/4001: Base Rate for Wheelchair Lift transport per One Way trip in the Medford, Klamath Falls, Grants Pass and outlying rural areas. CLINS 0002/1002/2002/3002/4002: Mileage Rate for Wheelchair trips from pick up point to destination. Rate is One Way in addition to the base rate. CLINS 0003/1003/2003/3003/4003: Base Rate for Gurney lift transport per One Way trip in the Medford, Klamath Falls, Grants Pass and outlying rural areas. CLINS 0004/1004/2004/3004/4004: Mileage Rate for Gurney trips from pick up point to destination. Rate is One Way in addition to the base rate. The Offerors shall submit copies of valid Oregon State driver's licenses for all drivers. 4.1.4.2. Offerors shall submit copies of current CPR licensees for all drivers. 4.1.4.3. Offerors shall submit a list of all vehicles with characteristics. 4.1.4.4. Offerors shall submit a copy of their Certificate of Insurance The selection resulting from this request for quotations will be made on the basis of the lowest-priced technically acceptable quote process from a responsible Offeror with acceptable or neutral past performance. Once the Contracting Officer has decided to whom an order will be issued, a purchase order will be issued using a SF 1449. Invoices will be submitted in arrears upon completion of services. In accordance with 52.212-3, Representations and Certifications, offers must either be accompanied by Representations and Certifications or the offeror must be currently registered in ORCA at website https://orca.bpn.gov/. The clause at FAR 52.212-4, Contract Terms and Conditions -- Commercial Items (Sept 2013) applies to this acquisition. The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders --Commercial Items (Jul 2010) applies, The following clauses within FAR clause 52.212-5 apply: 52.219-6, 52.219-13, 52.219-28, 52.222-3, 52.223-18, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.222-41, 52.222-42, 52.222-43, 52.223-18, 52.225-1 and 52.225-13. Other applicable clauses are 22.1012 WD 05-2439 (rev.-14) 52.204-9, 52.204-13, 52.217-8, 52.217-9, 52.232-19, 52.232-34, 52.232-99, 52.237-2 and 52.237-3. Applicable FAR provisions are 52.204-7, 52.212-2. VAAR clauses 852.203-70, 852.232-72 and 852.237-70 apply. VAAR provisions 852.233-71, 852.233-71, 852.270-1, and 852.273-74 apply.
