- RFQ Template Statement of Work This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This combined synopsis/solicitation is issued as a request for quote (RFQ)# F4D3B33318A901. Submit written offers IAW CLIN structure outlined in announcement. Oral offers will not be accepted. All firms or individuals responding must be registered with the System of Award Management (SAM) (https://www.sam.gov), formerly known as the Central Contractor Registration (CCR). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-70 and DFARS DPN 20131118. THIS PROCUREMENT IS BEING ISSUED AS A 100% SMALL BUSINESS SET-ASIDE under North American Industrial Classification Standard (NAICS) 561612 with a size standard of $19 Million. Project Number: F4D3B33318A901 Project Title: Night Monitor 1. Please see attached Statement of Work (SOW) dated 26 Nov 2013 for detailed description of required duties. 2. Quotes shall be valid until 9 Jan 2014. 3. Period of Performance: Base Year: 6 Jan 2014 to 5 Jan 2015 Option Year One (OY1): Period of Performance is 6 Jan 2015 to 5 Jan 2016. Option Year Two (OY2): Period of Performance is 6 Jan 2016 to 5 Jan 2017. Option Year Three (OY3): Period of Performance is 6 Jan 2017 to 5 Jan 2018. Option Year Four (OY4): Period of Performance is 6 Jan 2018 to 5 Jan 2019. Option to Extend 6-Months: Period of Performance is 6 Jan 2018 to 5 Jul 2018. Note: (Prices for 6-month extension shall be the same price as OY4.) 4. All questions or comments must be provided to the Contract Specialist (CS) and the Contracting Officer (CO) in writing via email NLT 4 December 2013. Answers will be posted NLT 9 December 2013. 5. The Government may extend the term of this contract by written notice to the Contractor within 30 days provided that the government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires.
