Request for Quote (VA786-14-Q-0036) To provide and deliver Certified TIF 419 Bermuda Sod to Fort Sam Houston National Cemetery NOTICE: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The Request for Quotation (RFQ) is VA786-14-Q-0036. No other solicitation will be issued in response to this announcement. This announcement will serve as both the synopsis and solicitation. Quotes in response to this combined synopsis/solicitation are due December 18, 2013 by 2:00PM MT and shall be emailed to Geraldine.Herrera@va.gov. OFFERORS CAN SUBMIT A RESPONSE TO THIS RFQ ON COMPANY LETTERHEAD. NO SF 1449 WILL BE ISSUED. Purchase order award will be made to the quoter/offeror whose quote is responsive, and offers the lowest aggregate price to perform the minimum quantities as specified in this announcement. This RFQ document incorporates provisions and clauses which are in effect through Federal Acquisition Circular FAC 2005-61. The associated North American Industrial Classification System (NAICS) code for this procurement is 561730 with a small business size standard of $7.0. This requirement is set-aside 100% for Service Disabled Veteran Owned (SDVOSB) Small Business Concerns; and only qualified and eligible SDVOSB concerns may be considered for possible purchase order award. The Delivery of SOD shall take place approximately 10 calendar days from receipt of purchase order award, and the period of performance is from date of award through September 30, 2013. NO INSTALLATION OF SOD IS REQUIRED. The place of performance where all work will be accomplished is the Fort Sam Houston National Cemetery located at 1520 Harry Wurzbach Road, San Antonio, TX 78209. Interested parties may contact the Cemetery 210-820-3891 to schedule a site visit if necessary. The contractor shall provide both a unit price and total amount for each of the following Contract Line Item Numbers (CLINS). The contractor shall provide all necessary supervision, labor, equipment, and supplies to deliver 100,000 square feet of certified TIF 419 Bermuda sod for cemetery staff installation. Contact the cemetery on projected time/month ahead of time for delivery. (See Scope of Work) attached. Offerors/quoters must be registered in the System for Award Management (SAM) at https://www.sam.gov to be eligible to receive purchase order award. In addition, SDVOSB concerns must be registered and have an active account in www.vetbizregitery.gov to be eligible to receive an award. The selected Offeror must comply with the following commercial services terms and conditions, which incorporates by reference: FAR 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition; FAR 52.212-3, Offeror Representations and Certifications - Commercial Items - the selected offeror must submit a completed copy of the listed representations and certifications; FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5- Contract Terms and Conditions Required to Implement Statues or Executive Orders - Commercial Items.
