Description

Vendor Questionnaire W-9 Scope of Work - HVAC 1. Emergency Repair of Heating and Air Conditioning units on the Gadsden Job Corps units. 2. Provide maximum of 100 hours of service for this contract. 3. The Center is only obligated to the extent of authorized purchases made under this agreement. 4. All shipments made under this agreement shall be accompanied by delivery tickets or sales slips that contain the vendor name, BPA number, purchase date, itemized list of supplies or services, quantity, unit price and extension amount of each item, less applicable discounts and delivery date. 5. The vendor will submit a consolidated summary invoice as a monthly billing for all deliveries made during the billing period. At a minimum, invoices must contain references to all BPA call numbers, an itemized list of the items delivered, unit and total costs, Center staff member authorizing the purchase and date of delivery. The invoice must be supported by signed receipt copies of the delivery tickets. Bid Closes at 5:00 PM December 20, 2013
