Description
The Federal Bureau of Prisons, Transitional Drug Abuse Treatment Contracting; Washington, D.C. is seeking quotes from sources having the ability to provide community-based outpatient Substance Abuse and Mental Health Services in Evansville, Indiana. Location of services should be within a 5 miles radius of U.S. Post Office located at 800 Sycamore Street, Evansville, Indiana 47708. Performance periods under this contract will be for a one-year base period to begin on May 01, 2014 with four (4) one-year option renewal periods with service ending (if all options are renewed) on April 30, 2019. The services required for this contract include a Base Year, Option Year One, Option Year Two, Option Year Three and Option Year Four, respectively. The Following services are required and may be sub-contracted to another licensed contractor. Base Year: Estimated Number of Inmates = 35 Substance Abuse Treatment Services: Intake Assessments & Report (one per authorized inmate) for 30 inmates = 30 assessments Individual Counseling Sessions (30 minutes = 1 unit) 48 units (per inmate) x 30 = 1440 units Group Counseling Sessions (30 minutes = 1 unit) 72 units (per inmate) x 30 = 2160 units Family Counseling (30 minutes = 1 unit) 8 units (per inmate) x 30 inmates = 240 units Individual Transportation (One Way) 104 trips for 35 inmates= 3640 trips Mental Health Treatment Services: Mental Health Intake Assessment & Report (one per inmate) for 05 inmates = 05 units Mental Health Individual Counseling (30 minutes =1 unit) 48 units per inmate for 05 inmates = 240 units Mental Health Group Counseling (30 minutes =1 unit) 72 units Treatment (TDAT) most recent Statement of Work dated April 2013- No Masters Required. The request for quote will be available on or after January 06, 2014 and will be distributed solely through the General Services Administration's Federal Business Opportunities web site http: //www.fedbizopps.gov. Hard copies of the request for quote will not be available. The site provides downloading instructions. All future information about this acquisition, including quotation amendments, will be distributed solely through this site. Interested parties are responsible for monitoring this site to ensure that they have the most up-to-date information about this acquisition. The estimated closing date is on or about February 05, 2014. Faith-Based and Community -Based Organizations can submit offers/bids/quotations equally with other organizations for contracts for which they are eligible. This requirement is unrestricted. The BOP anticipates awarding a firm-fixed price contract. All responsible business concerns may submit a quote which will be considered by this agency.
